PODIATRISTS IN THE COMMUNITY
CT Podiatrist Granted Advanced Ankle Surgery Permit

Jennifer A. D’Amico, DPM, fellowship-trained surgical podiatrist of Litchfield Hills Orthopedic Associates, has recently been granted an advanced ankle surgery permit by the State of Connecticut Department of Public Health. This new surgery permit enables Dr. D’Amico to expand her services to include a wider variety of advanced techniques and procedures to treat injuries and conditions of the foot and ankle. She sees patients at the practice’s locations in Torrington and Bristol.

Dr. Jennifer D’Amico

With the advanced ankle surgery permit from the State of Connecticut Department of Public Health, D’Amico will be able to perform ankle fracture fixation, ankle fusion, ankle arthroscopy, and other advanced ankle procedures. Prerequisites include the completion of a three-year, Council on Podiatric Medical Education-accredited residency program in podiatric medicine and surgery and satisfactory documentation of prior training and experience in standard or advanced midfoot, rearfoot, and ankle procedures.

MEDICARE NEWS
CMS Proposes Opioid Prescribing Limits for Medicare Enrollees

Pharmacies would have new limits on filling opioid prescriptions for Medicare beneficiaries under regulations proposed Thursday by the Centers for Medicare & Medicaid Services (CMS). Under the proposal, Part D plans would implement so-called "hard formulary levels" at pharmacies which would restrict the amount of opioids that beneficiaries could receive. "These are triggers ... [that] can prompt conversations between physicians, patients, and plans about appropriate opioid use and prescribing," Demetrios Kouzoukas, CMS deputy administrator and director of the Center for Medicare, said. "The trigger can only be overridden by the plan sponsor after efforts to consult with the prescribing physician." The safety level would be a 90 morphine-mg equivalent (MME) with a 7-day supply allowance.

The proposal also would limit the number of pills in an initial prescription for acute pain, "possibly with or without a daily dose maximum," he said, adding that CMS is accepting comments on the proposals through March 5. In addition, CMS "expects sponsors to implement 'soft' safety edits" to flag a potential problem, although those could be overridden by the pharmacist.

PRACTICE MANAGEMENT TIP OF THE DAY
Ways to Become a 5-Star Physician in Reviews - Part 7

7. Consistent Appreciation

"Thank patients at the end of the visit. One medical group we work with has focused on incorporating this into every visit this year and has seen their physician ratings move from average (3-star) to top performing (4.5-5 star). Many patients specifically comment on the 'thank you' message and how it differentiates them from their local competitors." CODINGLINE CORNER
Query: Modified McBride Coding

I am doing modified McBride procedures where I am removing the medial 1st met head,  performing both a capsulotomy and lateral release. What code should I use for surgery?

Doan Trinh Pham, DPM, Tracy, CA

Response: I would use CPT code 28292: Correction, hallux valgus (bunionectomy), with sesamoidectomy, when performed; with resection of proximal phalanx base, when performed, any method.

Please review this excellent article from Podiatry Management Jan 2017 for all the new coding changes in 2017 for bunionectomies:

RESPONSES/COMMENTS (MEDICAL-LEGAL)
From: Sean S. Ravaei, DPM

I get this request quite often from patients and I always decline. There is almost no evidence that this was caused by a concrete floor. I also get a lot of people asking me to fill out an application for DMV handicap placards. These are patients who are easily able to walk. I decline that too; they are both unethical. I see a lot of able people getting out of their car with a handicap placard. I think we as doctors need to do the right thing and stop being enablers.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Robert T Morris III, DPM

In light of the recent debate on practice valuations, I thought I might add my thoughts to the mix. I debated for a long while purchasing an established practice vs. opening outright before electing to open up on my own. Call me naive, but in today's world I do not see the value in what many of those selling established practices are offering. I get it, you worked your whole life to develop your nest egg into something worth selling upon retirement, but is the price you seek really worth it?

Let's start with facilities. Does your older office desperately in need of modern furnishing really command top dollar? We've all seen the surveys on PM News. Many older doctors are still...

Editor's note: Dr. Morris' extended-length letter can be read here.
