Cold Weather Presents Issues for Diabetic Feet: KY Podiatrist
 
Although anyone can have problems with their feet, the possibility of developing even a common foot issue, such as an ingrown toenail, can lead to infection or serious complications and even amputation for diabetics, Hardin Memorial Health podiatrist Russell McKinley, DPM, said. McKinley said patients with diabetes are prone to poor blood circulation and nerve disease in their extremities. As such, it is important they don’t solely rely on senses, but instead make decisions based on factors, such as a change in climate, to help maintain healthy feet.

When the weather becomes colder, or includes snow, rain, and slushy conditions, the potential for dampness exists. McKinley said moisture that collects between socks and feet and toes can form bacteria, which might lead to infection.

Touchette Regional Hospital has added Franklin Harry, DPM, a doctor of podiatric medicine, to its Archview Medical team in Sauget. Dr. Harry specializes in diabetic foot care, bone deformities, arthritis, and foot and ankle injuries.

Dr. Ravaei is correct. There are lots of malingerers who find the pathway to our offices. Dismissing everyone as being a faker is also not the right way to be. As a caregiver for elderly parents who need canes to walk to the curb, I appreciate the fact that some people indeed are handicapped and cannot walk distances. Each and every person must be evaluated as individual patients.
 
If a patient comes in requesting that we fill out a form for a DMV handicapped sticker and they need a walker or three-pronged cane to get into my treatment chair, they may have a valid request. On the other hand, if I am casting someone for "tennis" orthotics and that person asks for a handicapped parking permit, I will quickly show them to the door.

Generally, there are three different approaches that are used in the valuation of companies: the asset approach, income approach, and market approach. You can learn more about each of these approaches in APMA’s “Quick Reference Guide for the Purchase or Sale of a Podiatry Practice.” I also recommend participating in APMA’s webinar on “What to Know When Purchasing or Selling a Podiatry Practice.”

My “back of the envelope” way to evaluate a practice is the lesser of 1) the average of the last 3 years net annual income OR 2) 40% of the average of the annual gross over the last three years PLUS $1000 for ever new patient per month averaged over the last 3 years. If a gross is high because of high advertising, then the net will show the truth. If the overhead is tightly controlled or there is inordinate equipment expensed (those lasers and x-ray systems are costly), then the net will show the truth. 
 
Location (spare bedroom or medical center), equipment (even three lasers and digital everything), appearance of the front office, and everything else, is accounted for in the income and the new patients, already. New patients are the lifeblood of a practice, so a steady supply of new patients should be rewarded. But if all those new patients came because of heavy advertising, then this will be accounted for in the net value.

I once had a middle-aged male patient whom I was treating for some condition which required several visits. At the end of one of those visits, as I was walking past him to go out of the room, he called to me to ask a question. He was still in the treatment chair at a level below my vision. I glanced at the top of his head and was startled to see that he had supplemented the sparse hair on his head with black ball point pen lines (really).You'd be surprised how, at a quick glance, effective it was!
