Idaho Restricts Podiatrists Benefits
Recent changes to Medicaid in Idaho mean government is no longer footing the bill for some services. A memo from the state Department of Health and Welfare sent to podiatrists said that Medicaid will reimburse for preventative care or for treatment of acute foot conditions only if the patient has a chronic condition with vascular restrictions such as diabetes. The department's spokesman, Tom Shanahan, said changes to Medicaid are part of a statewide effort to save money on services that aren't mandated by the federal government.

Local podiatrist Dr. Brent Christensen said the changes in the law have been far from clear. "It was very confusing for myself and the patients. Patients didn't know, and I got a lot of phone calls, 'Is this service covered? is it not?'" Christensen said. But he said, in his experience, it hasn't been as bad as he first feared. "The number of people that we have discovered who don't qualify for podiatric services are relatively few and far between,” Christensen said. Other local podiatrists said they believe these changes will end up costing the government money, causing more people to go to the emergency room for treatment.

Spanish Podology Students Spend a Week at NYCPM
The New York College of Podiatric Medicine (NYCPM) recently hosted twenty-three podology students and several professors from Spain in cooperation with the Universidad Complutense de Madrid, providing them the opportunity to attend a challenging week-long course on anatomical dissection and surgical skills taught in the College’s state-of-the-art Bako Anatomy Lab. Michael J. Trepal, DPM, Vice President for Academic Affairs and Dean at NYCPM, directs this semi-annual program of visiting students from Spain, now in its twentieth year, on behalf of the College.

Professors Ricardo Becerro de Bengoa Vallejo, DPM, PhD, a graduate of NYCPM, and Marta Losa Iglesias, PhD, work closely with Dr. Trepal to direct the program. Both course directors accompanied the students on their visit. NYCPM has a longstanding affiliation with the podology community in Spain that was initiated by the late Professor Anthony Hernandez of NYCPM and is carried on in his memory. Teaching the students from Spain were professors Ricardo Becerro de Bengoa Vallejo and Marta Losa Iglesias, as well as professor Juan Carlos Carrera Merino and NYCPM faculty members Michael J. Trepal, DPM, Kevin Jules, DPM, and Anthony Dilandro, PhD. The course work involved lower extremity cadaver dissection as well as videotaped demonstrations.

Dr. Heather D. Driessen a native of Hardeeville has returned to open a podiatry practice in Savannah. Driessen earned her degrees and licensing as a Doctor of Podiatric Medicine and recently completed a three-year surgical residency at the Department of Veteran Affairs—Maryland Healthcare Systems in Baltimore, MD.

| IN THE COURTS | |
Appeals Court Strikes Down Individual Mandate in Reform Law
In a 304-page opinion, the 11th U.S. Circuit Court of Appeals in Atlanta on Friday struck down the individual insurance mandate in the Patient Protection and Affordable Care Act, but allowed the rest of the sweeping law to stand. In its decision, a divided three-judge panel of the federal appeals court ruled in favor of 26 states that had joined a lawsuit in Pensacola, FL which argued the reform law should be struck down because it relies on an unconstitutional expansion of federal power.
The ruling means that the Supreme Court will now have the classic split in the circuit courts that it often relies on when deciding whether to take on a case. The 6th Circuit Court of Appeals upheld the law in June, and the losers in that case filed for permission last month to have their case heard by the Supreme Court.

Docs Not Receiving Medicare Feedback: GAO
The Government Accountability Office recommended to the CMS in a new report (PDF) that the agency improve its process for giving feedback to physicians on their Medicare spending. The feedback, mandated by the Medicare Improvements for Patients and Providers Act of 2008, is designed to tell physicians how much Medicare is spending as a result of their care relative to other similar physicians.
But the GAO found in a performance audit conducted from June 2010 through August 2011 that most physicians were not receiving the reports after the CMS applied methodological screening criteria for determining which doctors are appropriate to receive the feedback. In addition, getting the physicians to read the reports was a challenge cited by the GAO report.

In turbulent times, people often are more hesitant to offer dissenting opinions or negative comments. One way to circumvent that problem during meetings is to appoint or vote for an “official” devil’s advocate. Rotate the assignment among team members to avoid dumping the burden on any one person.

Query: ESWT and NSAIDs
I performed a high-energy ESWT four weeks ago on a patient w/Achilles insertional tendonosis. Her edema and pain at the site are much improved as compared to pre-treatment. She wants to resume NSAID therapy for her knee arthritis (as prescribed previously by her orthopedist). Understandably, I am concerned if this will have any adverse effect on the ESWT site. I have read studies which suggest withholding NSAIDS for 6-12 weeks after ESWT. Is there a protocol for when patients may resume their NSAIDs after ESWT?

Query: Legality of Maximalist Running Shoes
Dr. Kevin Kirby described this shoe with, "I was very impressed a running shoe could be designed with this much cushion and spring." It's the "spring" portion I'm wondering about. When is a shoe illegal for competition because it gives an artificial mechanical advantage beyond a runner's biological ability? Are there any formal parameters by the running associations in this matter?

I attended Dr. Brody's lecture on EMR at the Annual APMA Meeting and he had the brilliant idea to provide clinical summaries on flash drives. Another perk was that a podiatrist could also market his practice on the flash drive.
I personally received my own medical info directly from the provider's contact website which was linked to an EMR.
