FL Podiatrist Discusses Causes and Treatments of Ingrown Toenails

“Contrary to popular belief, it’s not the nail that’s growing into the skin, but the skin that’s growing up over the nail,” says Dr. Amber Shane. Ingrowns happen when a nail is cut too short or its corners are rounded with a file, or when there’s too much pressure from shoes. Sports in which you frequently start and stop, like tennis or basketball, can jam toes and cause ingrowns.

Soak your foot in an Epsom salt bath to help relax the tissue, decrease inflammation and separate the nail from the skin a bit, Dr. Shane recommends. If it gets infected, see your podiatrist, who can remove just the part of the skin that’s ingrown. To prevent them, trim toenails straight across and leave them slightly longer than the end of the toe.

NY Podiatrist and Wife Named Co-Chairs of Hospital Black Tie Dinner

Washington Mills CEO Ron Campbell, Patty McPherson, and Anne & Dr. Richard Sawicki have been named co-chairs of The 2011 Premier, Niagara Falls Memorial Medical Center’s annual black tie dinner and entertainment gala. Dr. Sawicki is a podiatric surgeon and a member of Memorial’s medical staff.

The event will honor the Rev. Joseph L. Levesque, C.M., president of Niagara University and Ajit Trivikram, M.D., for their service to the hospital and community.

TX Podiatrist Runs in NYC Marathon

Local podiatrist Dr. Robert Klein joined 45,000 other runners for the New York City Marathon on Nov. 7. It was his second go at the event. He also ran it in 2006 and says it’s an experience unlike any other.

“There are two million people that line the streets of New York. There’s just nothing like it,” said Klein, who finished the 26.2 miles in a little more than four hours and 36 minutes. “There’s so much energy and a really cool vibe when you run through the City.”

MinXray Installs More Portable X-Ray/Digital CR Systems in Nigeria

MinXray Inc., a leading supplier in the portable x-ray equipment marketplace, recently installed seven more portable x-ray/digital CR systems throughout Nigeria, thereby increasing its presence in the healthcare arena in Africa.

The purchased systems included MinXray's HF120/60HPPWV PowerPlus, one of the most powerful portable x-ray units available today, and Carestream POC 140 CR computed radiography systems. The systems were installed in multiple locations around the country, in some cases where there was no existing x-ray imaging capability. System purchases were funded by the U.S. Government as part of the President's Emergency Plan for AIDS Relief (PEPFAR).

RE: APMSA Urges Profession’s Intervention to Avert Residency Crisis

Another option for graduating DPM students is to follow/shadow podiatrists in their community for however long it takes to obtain a residency position, and that could involve observing in the O.R. as well as the clinic. During that time, they could even prepare to take their Part 3 board exams. Also, they could do research, for however long it takes to find a surgical residency position. Many residency programs have fellows who do only research. Many of the masters of podiatry who are researchers could surely use a fellow, and I'm sure many of the residency programs which encourage research could open a pure research position. Coming from a residency program with a significant research emphasis (San Diego), I could certainly imagine doing research full-time, such as at the Temple school of podiatric medicine gait lab.

To the APMSA president, welcome to podiatry, where you pay dues and are obsequious to the APMA, yet they ask YOU for help starting residencies, instead of providing them. Where is the flurry of letters from the rest of the profession? Perhaps they're too busy writing out checks to the APMA, checks to the ABPS and/or the ABPPOM, state dues, continuing ed. seminars, state license renewals, DEA renewals, malpractice premiums, ad infinitum. Senior students - in about six months you'll be into it. Start being proactive early. It's your future, your money, your reputation.

I recommend the article by Ramirez MA, Rodriguez EK, et al., "Detection of orthopaedic implants in vivo by enhanced-sensitivity, walk-through metal detectors." JBJS(Am) 2007;89:742-746. Their conclusion: "More than half of all orthopedic implants may be detected by metal detectors used at commercial airports. Total joint prostheses will routinely set off the detector, whereas nails, plates, screws, and wires are rarely detected. Cobalt-chromium and titanium implants are more likely to be detected than stainless-steel implants."

This was obviously before the TSA switched to the full body scans that are currently all over the news. I often have patients ask this question, and in response I quote this study above. I have not seen a study that quantifies the amount needed to set off the metal detector. Nor have I seen a card given to patients by orthopedists who do total joints.

RE: Pain-killers Darvon, Darvocet Withdrawn at FDA Request

Vioxx, Avandia, now it's Darvon/Darvocet. What's next? As more careful clinical data is compiled and statistical analyses are completed, there will be many more drugs pulled off the market. What does this say about our RCT science? I know many of you have had a lot of negative things to say about holistic/integrative medicine, but the reality is that the protocols I use are not killing people. They are supportive to the immune system. It is when we use a drug to suppress a natural bodily function that we run into unpredictable side-effects.

It is also well documented that deaths from "properly" prescribed prescription medications are the cause of well over 100,00 deaths in this country every year (10% of which are from NSAIDs). There is huge momentum in the field of integrative medicine. Please do not tell me about all of the therapies that do not help people. I am not talking about irresponsible medicine (which certainly comes into the mainstream as well). I am talking about well studied doctors who apply their craft diligently and appropriately. The time is now to acquaint yourselves with the new paradigm.
