Query: Fall Risk Assessment Under MACRA

 

I am trying to consult different sources for understanding and performing the fall risk assessment for MACRA. There are so many ideas and sources. Is there is a standardized form that is available which makes this a little easier to understand? I would like to be certain that I am doing this properly.

Query: Registry for MIPS

 

What registries are available to attest for MIPS for 2017? My EMR does not integrate with the APMA's registry.

Query: Reporting MIPS without EHR

 

I want to participate in MIPS reporting to CMS to avoid receiving the 4% fee reduction. The problem I am having is that I am not on an EHR system. I was told by my practice management software, Healthfusion/ Meditouch, that without having EHR, I cannot report findings. I only want to do basic reporting of patient tobacco use. Does anyone have any advice on how I can report this to CMS?
