NY Podiatrist Comments on Gross Medical Video


 


A video posted to Reddit will captivate even the most jaded fans of gross medical videos. To get an expert's take on the mysterious object, INSIDER spoke with podiatrist Dr. Hillary Brenner. She stressed that it's impossible to identify the object from a video alone, but she  suggested it could be a wood splinter.

Brenner told INSIDER that she has seen foreign objects puncture the skin of the foot in one place and come out in a completely different location. Though she hasn't personally witnessed an object come out through the top of the foot, she said it was possible this particular thorn/splinter went through the bottom of the foot and worked its way to the top. She also took note of the golden pus that's visible near the 1:07 mark. "That's [a sign of] an infection that  has been brewing," Brenner said. "It has been in there for a long time."

Although I tend to agree with an aseptic procedure, I think that Dr. Gurnick’s criticism is overboard. This is an infected procedure, beyond "contaminated" and "dirty" .The likelihood is that the wound is way more contaminated than might be the significant value of field and skin prep. I am delighted that the video was published on PM News. I liked the technique of carefully extracting the object rather than one quick movement.

I do not know who the person was who removed the foreign body  (podiatrist, other doctor, nurse, etc.) but please, please, please to all podiatrists, no  matter how simple a procedure may appear to be, please use sterile instruments, do a cleansing and prep the site, drape the foot, and make sure at all times that you wear gloves. The technique seen in this video is below the standard of care in 2018.  


 


There is an obvious foreign body and abscess. Take a proper wound culture. If there was a bad outcome in this situation and along with the video of this technique for removing a foreign body, the lack of sterility would be difficult to defend in a malpractice case. Also, I suggest that you stop allowing patients to video procedures in your office. It can only come back to hurt you later.
