The “new” bunion procedure that is called the lapiplasty which was brought to fruition by the Dr. Paul Dayton, et al. and his team certainly has “changed” the way we look at the biomechanics of a bunion deformity, let alone, how correct to it.
But, the question I pose to the PM News readers and to my podiatric colleagues is this bunion procedure the pinnacle of podiatric surgical success because of its continual outcome of “reproducible” results along with reduction in bunion deformity reoccurrence?
Or is this procedure driven like most our profession’s innovations by the vendor dollars created by performing said procedure?
Or is this procedure’s ability to stay in the limelight of surgical success fueled by the seminar revenues created in the process of getting our profession all the same page on correcting this pedal nemesis?
These questions need to be answered.
I understand that this profession has evolved surgically over many decades and most likely will continue to do so, but it seems to me that this profession loves to attach itself to something by marketing it as “the next best and greatest thing” that will change the face of podiatric medicine forever.
We know that is not true. Look at all the “waves” of podiatric innovation through the years that we have rode on until they finally crash into the shore. We seemingly never ride that “wave” again.
Can anyone say “Extracorporeal Shockwave Therapy”, “Laser Treatment for Onychomycosis” or how about “Endoscopic Plantar Fasciiotomy” to name a few?
How much money was made from these “waves” of temporary success and at what cost? Did all the patients who had these procedures performed have “reproducible” outcomes and “predictable” results? Anyone? Buehler? Buehler?
I also understand that much “research” along with a lot of opinions on this procedure have been published in the podiatric literature which have legitimized the procedure’s necessity, profoundness, and proficiency.
But, being that this bunion procedure has only been around for a few years how is it that such a rush to judgement can be made regarding these surgical and foundational outcomes?
I always understood solid research to include a specific amount time (5-10 years) along with a high sampling of patients (hundreds to thousands) before any credible statements of procedural or treatment outcome could be made.
Therefore, my concern is that certain individuals who are performing this procedure and who may have a financial interest in its long term success are touting “news” to press to sensationalize a story about “new bunion procedure” which is not “new”, but rather nothing more than a way doing a bunion procedure differently.
Look in any podiatric surgical textbook and you will see the litany of bunion procedures that all had their claim to fame at one time or another. And if you have been around long enough they all were “new” at one time, but alas most have been relegated as ideas that have failed to produce credible and “reproducible” results. Hence, the rediscovery of the Lapidus and this “new” modification.
So, before we all jump on the band wagon of doing this procedure let’s remember that a hammer is not the only tool in the tool box.
A final thought is that I also find it interesting is that if wasn’t for Dr. Dale Austin the majority of “reproducible” bunion surgery with successful outcomes that we perform would be named something completely different.

1. Preparation is Key - Spend some time the night before familiarizing yourself with the surgery. While you shouldn't sacrifice sleep, investing an easy 15 minutes in reading resources like OrthoBullets, McGlamry, Coughlin, or relevant articles can significantly enhance your confidence and ability to answer questions correctly. Do not only sleep 2 hours the night before surgery to prepare… you need sleep. Balance is important.

5 Practical Tips to Reduce Denials, Improve Cash Flow
Technology is often the go-to solution for healthcare administrative workflow woes, including billing. Still, it’s just one of several practical steps that can deliver tangible impact, many of which require little to no capital investment. Here are five of the most effective.
1A. Monitor KPIs - Implement a mechanism to monitor key performance indicators (KPIs) to track the practice’s revenue cycle management (RCM) effectiveness and efficiency and identify bottlenecks that hinder cash flow. Financial KPIs show how well the practice is converting services into income. They include Net Collection Rate, which should run 95–100%; Gross Collection Rate, which is helpful for spotting capture issues; Average Reimbursement per Claim, indicating payer performance and coding accuracy; and Bad Debt Rate, which signals poor collections or communications when write-off rates are too high.
Operational KPIs reflect workflow efficiency by highlighting how smoothly the billing process runs. The Clean Claim Rate should ideally stay above 90%, while Days in Accounts Receivable (A/R) should be under 45. Also good are a short Claim Lag Time, low Denial Rate, and high First Pass Resolution rate.

Of the 28,000 runners, podiatry services treated over 40 injured runners. According to Dr. Ross, many of the injuries treated were a result of runners who wore lightweight mimimalist shoes with a rocker sole design.

NJ Podiatrist Discusses What to Look for in Winter Footwear
Finding a truly good pair of winter boots or shoes is trickier than it sounds. The best winter footwear has to check a lot of boxes: it should keep your feet warm and supported, provide solid traction and, ideally, offer waterproof protection for slush, ice and snow. “When shopping for winter shoes, it is important to consider options that keep your feet and body aligned and provide proper arch support to reduce the strain on both your plantar fascia and heel,” says Janine Ferrigno-Taddeo, DPM, a podiatrist at Millburn Podiatry in New Jersey and liaison to Aetrex footwear.

Her advice: Look for options that are lightweight with an ethylene-vinyl acetate (EVA) midsole, a lightweight, flexible blend of plastic and rubber. “This provides cushioning and shock relief,” says Dr. Ferrigno-Taddeo. Adjustability is also important so you can tailor the boot or shoe to the level of support and security your foot needs.

Dr. Francis ("Fran") A. Kania of Lombard, IL, passed away peacefully at home, surrounded by his loving family, on January 8, 2026, at the age of 65 after being unexpectedly diagnosed with glioblastoma.

Fran received his doctorate from the Dr. William M. Scholl College of Podiatric Medicine in 1988. Fran practiced as a podiatrist for 36 years and truly loved his profession. He cared deeply for his patients, whom he served in the Westchester and La Grange Park communities.
