The doctor who has a patient with "unrelenting
metatarsalgia" has relief with a wheal raised
just over the posterior tibial nerve. This
relief last about 5 days.

Whenever I had a patient who experienced
unrelenting nerve pain, I recommended looking
for a systemic condition that causes the pain.
There can be many of them, and usually they
require a work-up combined with a referral to a
neurologist to help look for that explanation.
It is sometimes too easy to get caught up in
local podiatry without keeping a necessary
wider perspective.

All specialists have a tendency to do this, not
just "limited licensed" doctors. Systemic
conditions like lupus, MS, nerve poisoning by
exposure to heavy metals, mono-neuropathy,
incipient diabetes, regional pain syndrome,
even early phase ALS may be considered. There
might be an "atypical" presentation of a
systemic disease involved. Although many nerve
conditions present with muscle weakness and
fasciculations as a starting point, not all
do.

There is such a thing as a "maturing
diagnosis." This means that some atypical
phases of generalized disease take a while to
develop symptoms that finally point to the
cause. The "end person" who finally identifies
it is considered a medical hero. In truth, it
is often a matter of just "waiting" until the
real symptoms of the disease develop.

This patient appears to have had at least
"some" general medical work-up. But even MDs
miss things. In addition, biomechanical
features should be considered, which definitely
includes ruling out equinus, which puts
inordinate pressure on the forefoot while
walking. All DPMs should be checking this out
in their patients, anyway. If the patient has
severe equinus, local treatment, even with
orthotics, etc. will not help, except possibly
temporarily.

GA Podiatrist Appointed to the State Board of Podiatry Examiners


 


Governor Brian P. Kemp has announced that he has appointed Rupal Patel Gupta, DPM to the Georgia State Board of Podiatry Examiners. Dr. Gupta earned her doctorate in podiatric medicine from the Kent State University College of Podiatric Medicine and completed her residency at Jackson North Medical Center in North Miami Beach, FL.

Actually, military podiatrists are NOT members of the Medical Corps in the armed forces. USAF podiatrists are in the Biomedical Services with all the other non-physician healthcare providers except dentists (who have their own Dental Corps). The U.S. Navy and U.S. Army podiatrists are in the Medical Services Corps.  


 


The addition of podiatrists into the Medical Corps has been an ongoing concern since the beginnings of podiatrists in the military. That is why a degree change is the only way for a DPM to be commissioned into the Medical Corps. This issue is extremely important as to the acceptance of DPMs as physicians.

RE: Medicare Advantage (MA) Legislation in Congress: 7 Bills to Know

In  a recent Becker's Payer Issues there is a must read article for every doctor in the country. There are 7, that's right, 7 bills before Congress which will directly impact MA plans. One will require MA plans to pay the same as the fee for service Medicare. It actually has bipartisan support. There are a myriad of others which address prior authorization issues, data mining (several would actually outlaw it), and others. 


 


If the proposed regulation requiring them to pay the same as the fee for service contractors is implemented, and the MA plans are taking the risk, how long do you actually think these plans will hang around? If the bill is enacted, how often will these claims be lost or never processed for a myriad of reasons by the MA plans? Perhaps legislation needs to be enacted along with this first proposed law which requires MA plans to pay clean claims (in accordance with Medicare Fee-For-Service) requirements.

It’s true that a good lab probably cannot make custom orthotics for 45 dollars. There is, however, a greater issue behind this problem. Why are so many of our colleagues attracted to labs that are charging 45 dollars for "custom" orthotics?


 


Several years ago, when I was casting for and dispensing  a great deal of orthotics, I ordered a set of custom tennis orthotics for a patient. He agreed to pay me for them. They addressed his problem nicely and all was well until I received a call from his wife and the insurance company. They informed me that I am contractually obligated to accept only what the insurance company is willing to pay.


 


Bottom line: I had to refund what the patient paid me and agree to accept twenty-six dollars and fifty cents for the orthotics. I lost money, but learned that with most of my insurance carriers, I would no longer be able to cast for and dispense orthotics. I consulted with the APMA insurance expert and was bluntly told that if I opt out of the contract, I could kiss all other podiatric services goodbye. I had to make a hard choice.

As a fellow Nebraska podiatrist and Scholl graduate, who has known Rick since 1977 when I opened a practice in nearby Omaha, I can vouch that Rick was a pioneer for the profession. He had to fight "tooth and nail" for privileges and helped to update an antiquated scope of practice law for our state. He practiced with integrity and was always a credit to our profession. RE: The Passing of John Timothy “Tim” Stivers, DPM


 


Dr. John Timothy “Tim” Stivers, beloved podiatrist, humorist, performer, and pilot, has passed. Known for his quick wit and unforgettable storytelling, Tim brought joy to countless people through his unique sense of humor, colorful dialects, and “good ol’ boy” stories. Tim didn’t just tell funny stories, he “told stories funny”. A gifted entertainer, Tim was often featured on local and regional television and radio shows.

Born and raised in Anchorage, KY, Tim followed in his parents’ footsteps and became a podiatrist. He graduated from the Illinois College of Podiatric Medicine in 1958. He liked his medical practice, but he loved singing, flying, and making people smile. His impeccable timing, laughter, and larger-than-life presence will be deeply missed.

MEDICARE ADVANTAGE


UnitedHealthcare to Exit Certain Medicare Advantage Markets


 


UnitedHealthcare CEO Tim Noel offered investors a deeper look at the medical cost spike that's plaguing the insurance giant's finances. He said during the company's earnings call that pricing assumptions set by the company "were well short of actual medical costs" for 2025. UHC's current outlook, he said, instead reflects an additional $6.5 billion in medical costs, with more than half, or about $3.6 billion, coming from its Medicare plans.


 


Noel said that in MA specifically, the team is looking to adjust pricing and benefit designs to account for the cost pressures, which they anticipate will stretch into much of 2026. It has also decided to exit certain markets largely with plans that are more loosely designed, such as PPOs, in a move that will impact 600,000 beneficiaries.

A neurosurgeon woke one morning to find around 6” of water flooding his basement. He called the local plumber and told him what was happening, and the plumber said he’d be right over. When he arrived, the neurosurgeon led him to the basement. Donning his waders, the plumber stepped into the water. Leaning over, he felt around for a few minutes, pulled the drain plug, and said, “There you go, it’s fixed”.  “How much do I owe you?” The plumber said, “That’ll be $400, cash, check, or credit card”. “Wow, I’m a neurosurgeon and I don’t make that kinda money!” The plumber smiled and said, “Neither did I when I was a neurosurgeon!”
