CA Podiatrist Not a Fan of Shoeless Runing

Caity McCardell was running along the Huntington Beach bike path when she had hit a rocky patch of the blacktop.  She was running barefoot.

 Dr. John Pagliano, a Long Beach-based podiatrist who specializes in sports injuries, is not a fan of running shoeless. "If you're 150 pounds, whether you come down on your heels or your forefoot, it's still 150 pounds," he said. "I think you really need the shock absorbers from the shoe and control of shoes while running."

APMA HOD NEWS

APMA HOD Begins

The 91st Annual American Podiatric Medical Association’s House of Delegates (HOD) began today in Washington, DC. In the morning, the Resolutions Committee, chaired by Tim Tillo, DPM, considered 17 proposed resolutions. The committee recommended “do pass” for 15 resolutions, with two resolutions being withdrawn.

Among the notable resolutions was a proposed dues increase of $50 for the fiscal year starting in June, 2011, with subsequent $25 increases in 2012 and 2013. Another resolution paved the way for removing the limitation on the number of Board of Trustee Members who can come from a particular state. All resolutions will be considered by the full HOD on Sunday and Monday.

MA Podiatrist Faces MS with Positive Attitude

When Jeffrey and Janet Bernardone were married in June 1990, they began living the American dream. Their son, Jeff, was born in 1991 and the family moved to Mansfield when Janet was pregnant with their daughter, Marissa, in 1995. Things changed during that move. Jeffrey’s legs became very weak and he would collapse at times and then recover. Visits to New England Medical Center (Tufts) brought the diagnosis of relapsing-remitting multiple sclerosis (MS).

Jeffrey, a former podiatrist, stopped working as his disease progressed to secondary progressive MS. He now has no use of his legs or left arm, minimal use of his right arm, double vision at times and also is hard of hearing. Yet he is grateful, not for the disease but for what going through the disease has brought out in family and friends. “There are other people worse off,” Jeffrey said. “I’m grateful. I have two beautiful kids and I am at home. MS is in (my) cards and (I) have to learn to deal with it.”

When asked the mission of Stajo Construction, Stan and Josh Rosenbaum reply in unison, "Our goal is to satisfy the customer." Father and son, respectively, say they accomplish their goal by combining a commitment to meeting their clients' individual needs while providing high-integrity housing delivered within budget and on time.

Dr. Joshua Rosenbaum, now 37, was a biology major who got his doctorate in podiatry. But his passion for building and environmental issues lured him back into the industry. "About nine years ago, I took a year or two off to work full-time with my dad, mainly working on site with the superintendents," he recalls. "In 2008, I got my general contractors license from the state of Florida. I'm very interested in green construction and development. So I became a certified green professional through the National Homebuilders Association, as well as an accredited professional with the Green Building Council.

Query: Pain in Hallux Interphalangeal Joint (Jordan Sheff, DPM)

It's likely that the MPJ is not dorsiflexed sufficiently to allow for propulsion and as a result, there is now  jamming and increased mobility at the hallux IPJ. While this may sound radical, I had a similar case sent to me a number of years ago where I recut the MPJ and inserted a total silastic implant. Since the fusion appears solid, you can determine at what level you wish to recreate the MPJ so that the first metatarsal length can be preserved.

The surgery has placed abnormal forces on the distal joint. Try injection of steroid with rigid soled shoes and NSAID if tolerable. If this fails, I recommend an arthrodesis of the joint.

It appears that this patient may have an increased amount of keratin in that area of her toes – think hyperkeratosis. Keratin absorbs several times its own weight in water, like a sponge. I’m sure readers have seen calluses turn the same white in patients with hyperhydrosis.

Dr. Mayer - you write in your posting that your patient tested negative for Raynaud's. What test are you referring to? To my knowledge Raynaud's is a clinical diagnosis, I was not aware of any "test" for it.

Panafil  (as well as Accuzyme and several other papain/urea-based debriding agents) was taken off the market in 2008. As far as I know, the only currently available active enzymatic debriding agent is Santyl (collagenase), which is manufactured by Healthpoint.

I, and many of my fine colleagues, "feel your pain." The question, as I humbly see it, is whether we as a group of physicians can collectively bargain. Currently, and unfortunately, the answer is NO because of the Sherman Anti-Trust legislation. In my opinion, the utilization of PACs through state and national associations to promote "collective bargaining" for physicians in this current political environment would probably NOT be effective.

I believe there is no appetite by the public or most politicians to extend "collective bargaining" to...

I would like to ask Mr. Mauss, and your point is...? The definition of welfare is clear in many dictionaries: health, well-being, prosperity. The word has been in use since the Middle Ages, so you think it was used in a different context when the U.S. Constitution was drafted?

If a government has the power to excise taxation from its citizens to contribute to the general welfare and security of the country they reside in, it also should have the duty to provide such welfare in the way it sees fit, when appropriate, and in a legal manner for the benefit of the citizens of such country.

Now leave it to the hundreds of lawyers and amendments that have been passed in the last 200 plus years, and what has been created is a Pandora's box of irregularities and self-interest benefits.

RE: Homeopathy is a Safe Way to Complement Treatment Choices (Robert Steinberg, DPM)

Dr. Steinberg's assertion that modern homeopathic treatment is based on the "pseudo-science" of long ago is entirely biased and without education in this field. The use of homotoxicologic preparations is based on modern science and an understanding of the effects of toxins on the extracellular and intracellular matrix. Dr. Steinberg and others like him are welcome to keep their heads in the sand, but the reality is that when used properly, these medicines effect cures that are quite profound and observable. This is a growing field not because there are so many doctors who believe it works, but because it does work and patients are realizing the benefits of it and demanding it more and more.

As I have said countless times on this forum, it is not a stand-alone treatment, but part of a bigger protocol designed to optimize the health of the patient and the reactivity of the immune system. The 19% who responded to PM's survey favorably about homeopathy will be able to create amazingly successful niche practices in integrative medicine, while the rest will continue to struggle with managed care control and shrinking incomes.
