OHFAMA Meets with OOA Board of Trustees
On Saturday, March 1, the Ohio Foot and Ankle Medical Association (OHFAMA) executive committee met with the Board of Trustees of the Ohio Osteopathic Association (OOA), and the Dean of Ohio University’s Heritage College of Osteopathic Medicine.

Issues discussed were the HELLPP Act; the decline of solo practitioners; DPM education, residencies and training; scope of practice; demography of DPMs in OH; DME issues and diabetic collaborations; and vascular disease prevalence in Ohio. After the presentation, OHFAMA was invited to a reception with the OOA board members and spouses.

FL Podiatrist Awarded Patent For Simplified Electronic Billing System
Dr. Steven Kaniadakis has been granted patent US 8,666,772 by The United States patent Office. The patent, issued March 4, 2014, is titled "Process, System, Method Creating Medical Billing Code Letters, Electronic Superbill and Communication."

This invention is a method, creating a machine-readable language of medical billing codes from medical records by artificial intelligence and human intelligence. The software-related engine is achieved by converting difficult billing code numbers into easy code letters. Also, this software makes scheduling easier, with re-appointments, and sends letters by electronic means, messaging, voice-over-Internet protocol, and wireless connections using a unique implementation of other applications in combination with this novel medical software operation.

Query: Erosive Heel Lesion
This 59 year old female developed sudden onset, severe heel pain without any history of trauma. Pertinent PMH includes obesity and DVT. Physical exam demonstrates severe pain on palpation of the calcaneal tuberosity. There is no bruising, erythema, or increased skin temperature. X-rays reveal multiple circular appearing erosions in the inferior calcaneal tuberosity with moderate adjacent spurring. Bloodwork: rheumatoid panel was negative except for elevated uric acid of 7.5. MRI showed a large scalloped erosion in the calcaneal tuberosity with adjacent marrow edema and a “lobulated ill-defined adjacent soft tissue lesion” with tearing of deep fibers of PF per radiologist’s report.

I attempted aspiration of the area (thinking that there may be uric acid crystals) without success. The patient has no insurance and cannot afford an expensive open biopsy. Any thoughts as to whether a first-time gout attack could produce erosion like this? The radiologist mentioned differentials of enthesitis with inflammatory pseudomass (most likely), OM, or soft tissue neoplasm (least likely) as possibilities. Suggestions?

Digit gigantism can be a clue for neurofibromatosis (Von Recklinghausen's disease). Check the entire body for cafe-au-lait spots. Try to elicit a family history. Also, when there is one overt defect, it may signal internal embryonic mis-development. Heart and kidney malformations may be concomitant. An assiduous pediatrician would be your best ally in investigating this.

Healthcare advisors to hospitals and big medical businesses have been telling them to prepare to not get paid for six months after ICD-10 goes into effect. Many of you involved with these systems may be noticing the cuts they have been making to staff, including physicians.
If this happens, many of you will lose your jobs. Most private practice people will survive this. I suggest having all your Medicare patients sign an ABN after October 1st. If Obamacare is not delayed and...

Years ago, I had a similar situation when the patient, to my surprise, started to record my pre-op consultaion on a tape recorder. I decided on the spot NOT to perform the surgery since I felt that the mutual trust was breached.The patient was very apologetic and was very eager to have the surgery, but my gut feeling told me that this patient was a potential problem. I know that many readers will disagree, but I am convinced, based on impression only, that I did the right thing.
