Dr. David Samuel of Springfield, PA serves as a preceptor to the Department of Family Medicine of the Crozer Keystone Health System. He lectures to the family practice residents on all aspects of lower extremity care. Many of the family practice residents choose to do office rotations with Dr. Samuel as well.

He recently conducted a workshop where he lectured to the residents about podiatric medicine and also demonstrated and instructed them about typical podiatric office procedures.

MI Podiatrist Runs Shoe and Boot Drive for Homeless
Dr. Anthony Weinert, a podiatrist with offices in Troy and Warren, will be running a winter shoe and boot drive benefiting Grace Centers of Hope, Oakland County’s largest and oldest homeless shelter and charity providing food, clothing, and hope to disadvantaged individuals. The purpose of the shoe and boot drive is to help those individuals facing hardships in their life to have adequate protection for their feet as cold winter months loom in the near future.

The drive will run from November 18th through December 22nd, with shoes being collected at Dr. Weinert’s offices and other select locations. Shoes will be distributed to Grace Centers of Hope on the 23rd, to benefit homeless and disadvantaged individuals before Christmas.

NY Podiatrist Discusses Fashion Shoes
Are some heels worse than others? I asked an expert, Dr. Robert Conenello, a podiatrist from Orangeburg, NY. "Ones that are over two inches and have very tight toe-boxes. The most fashionable shoes show 'toe cleavage'...the space between the first and second toe. These have the least support," says Conenello.

NY Podiatrist Receives NHCAA 2013 Medical Director Award
The National Health Care Anti-Fraud Association (NHCAA) has named Gary Cicio, DPM as its 2013 Medical Director Award recipient for his significant contributions and outstanding accomplishments to the field of healthcare fraud-fighting.
For more than 18 years, Dr. Cicio has served as an independent consultant, and more recently as a clinical director of fraud investigations for WellPoint, Inc., a member of NHCAA. In his consultant role, he contracted with many regional and national health plans and program integrity organizations nationwide, and worked with federal and state law enforcement providing pre-trial case development and expert witness testimony in both civil and criminal cases.

NY Podiatrist Pleads Guilty to Distributing Oxycodone Charges
Two Long Island men pleaded guilty Wednesday to illegally distributing prescription drugs, federal prosecutors said. Rools Deslouches, 42, a nurse practitioner from Brentwood, and Dr.Stephen Owusu, 57, a Valley Stream podiatrist, each pleaded guilty in unrelated cases at federal court in Central Islip to providing oxycodone illegally, said Loretta Lynch, U.S. attorney for the Eastern District of New York. Owusu, whose practice is in Brooklyn, wrote more than 400 prescriptions for the pills, several to undercover agents in a Rockville Centre parking lot, according to court papers.

Query: Unusual Skin Fissures
This 50 year-old male patient suffers from chronic patches of fissuring skin as noted below. His medical history is significant only for heavy alcohol use. He admits to frequent barefoot walking and use of sandals. Shaving of the skin was negative for dermatophyte infection on PAS stain.

The patient has, to this point, refused a punch biopsy. Any thoughts on etiology and appropriate treatment would be appreciated. The fissures are very painful for the patient.

Query: Starting a Wound Care Center
I have been approached by some physicians about being a partner and supplying the facility for a new wound care center. Although I have started a single specialty surgical center, I am fairly neophytic regarding the issues surrounding start up of a wound center. Is there anyone that you would suggest I contact who has experience and might give me some advice in this potential endeavor?

When referring a patient on to a physician of a different specialty, I give them more than one name (I believe 3 is the norm). I also tell them to go back/call their family physician for additional names/referrals if there is a specific doctor they work with, and to keep them in the same health system for continuity of care and coordination. Yes, I would document this in the patient record and have the patient keep me updated at a follow-up encounter.
I will qualify my statement by stating I am not a healthcare attorney. I opine one is not responsible for the treatment outcome another surgeon achieves. I also believe that we are indemnified against any negligence that might be alleged against another healthcare provider. Additionally, it's a patient's right, and OPTION to either follow your recommendation, or not. In good consciousness, if you feel a given medical/surgical issue is best served by referring to a specialist, then do so. Scope of practice doesn't seem relevant.
If the issue were misdiagnosed and the patient was incorrect in what condition they thought they had, it is likely that specialist would refer to the appropriate one after evaluating the patient. Document the patient's request in the subjective portion of your SOAP note, and your recommendation/referral in the treatment plan. I just made a similar referral yesterday for a friend of mine who asked for help dealing with the EXACT same pathology.

Dr. Kosova had an incident where his server motherboard failed and required replacement. Without the server, he did not have access to his EHR, his appointment schedule, and his digital x-ray system. Under the HIPAA rules, each of us must have a documented plan on how we deal with 'unscheduled downtime' - that is time when we do not have access to our computer systems. We also need to have a disaster recovery plan.
The disaster recovery plan needs to include the proverbial "Whom do you call," along with the steps necessary to get back up and running. We should also test our disaster recovery plan to make sure that it actually works. Based upon his post, it is clear that Dr. Kosova's disaster recovery plan did work. Well done, Dr. Kosova.
The same situation could happen to your practice. If it does, do you have the contact information for everybody that you need in one place? Do you have appropriate 'paper forms' on hand to utilize during the time your EHR is down? Your HIPAA security manual should have sections that document how you would deal with a situation like this. Now is a good time to look at your manual and make sure that section is up-to-date. If it is not, then it is time to update your HIPAA security manual.

Dr. Glenn Gastwirth deserves to be acknowledged for delaying his retirement until 2015. It is said that no one is replaceable, but in this case, it may not be so. I have worked with Glenn on projects over the years and found him to be totally hands-on when needed. He edited our journal before Dr. Warren Joseph took over. He ran our national conventions until he was able to appoint great people to take on that responsibility. The list of what he has done for the APMA is vast. Glenn Gastwirth's most challenging project may very well be finding a successor who can come close to his talents.

I believe the story regarding balance in the elderly could have been better stated to more positively reflect on the profession. While we certainly should unashamedly profess our academic and practical knowledge along with our technical skill in treating our patients in the context of their footwear, if we want to elevate the profession and be considered physicians, we need to refrain from reflexively talking about shoes as soon as we speak.
There is so much more that could be said regarding the lower extremity and balance that is consistent with a physician's level of expertise. When responding to questions, we need to think about moving footwear down the list. Physicians would speak first about the lower extremity in the context of the patient as a whole. Then, when applicable, consider the lower extremity in the context of footwear. In this case, we are less apt to sound like glorified shoe technicians. It is time to slay the shoe dragon and better represent our knowledge, the physician status we aspire to, and the three years of post-graduate training we now demand.
