IL Podiatrist Discusses Turf Toe and Sprain
 
Is it turf toe or a sprain? Here's how Dr. Michael Chin, medical director of The Running Institute and Windy City Foot & Ankle Physicians, explained the difference: "You can sprain it the opposite way where the toe bends downward and you have a sprain on the top of the capsule. Depending on the severity, it could end up being similar to turf toe. But typically turf toe is the bending up of the joint and the pain is on the bottom of the toe."

Chin, a doctor of podiatric medicine, said turf toe can take four to eight weeks to heal, although he allowed that a world-class athlete could return sooner.

AT THE COLLEGES
NYCPM and National Univ of Ireland Move Forward on Educational Initiatives 
 
A recent visit to the National University of Ireland in Galway (NUI Galway) by Michael J. Trepal, DPM, Vice-President for Academic Affairs and Dean at the New York College of Podiatric Medicine, marked the next step in the implementation of a Memorandum of Understanding reached between the two institutions in 2011.  
 
(L-R) Ms. Claire MacGilchrest, Ms. Amanda Walsh, Michael J. Trepal, DPM, Ms. Elaine Hyslop, Mr. Nigel Roberts, and Dr. Caroline McIntosh, Head of the Discipline of Podiatry at NUI.
 
During his visit, Dr. Trepal met with both the Dean of the University and with Dr. Caroline McIntosh, Head of Podiatry, School of Health Sciences, as well as with members of the faculty. Initiatives that are about to be instituted include a student exchange in March, shared research and continuing medicale education, and videoconferencing of grand rounds and other academic classes and activities.

OCPM Student Affected by Wikipedia Blackout
 
This week, Wikipedia, went dark in protest to piracy legislation. From time to time, Aaron Utterback comes across medical terms he's not familiar with when studying for podiatry boards. If that happens, he knows where to turn. "Wikipedia is a great way to get the basics of concepts, so I use it all the time and my classmates use it all the time as well," Utterback said.

But today, when he needed to look up "calcium channel blockers," the site didn't work. "It pops up for just a second and then leaves and then blacks out to the screen that says we are doing the 24 hour blackout," he explained.

WV Podiatrist Voluntarily Surrenders License
 
Dr. Seth Stinehour has voluntarily surrendered his podiatry license relating to engaging in unprofessional conduct and the inability to practice podiatry with reasonable skill and safety due to mental impairment. 
 
A consent order entered by the board accepts the surrender of Dr. Stinehour's license. Dr. Stinehour will not apply to the board for licensure until he proves by medical and psychiatric evidence, to the board's satisfaction, that he is competent to return to the practice of podiatry.

FPMA SAM 12’ Meeting is Another Unqualified Success

While much of the country was dealing with harsh winter weather, over 750 podiatrists (many from outside of Florida) enjoyed sunny skies and warm temperatures at this year’s FPMA Scientific and Management (SAM) meeting. Attendees were treated to a spectacular mix of world-class scientific and practice management presentations, as well as fun-filled functions. Over 150 exhibitors displayed the latest state-of-the art products for the profession.

QUERIES - CLINICAL
Query: Integrative Nerve Blocks
 
I have had some patients come into my office relating that their primary care doctors are sending them to pain clinics that advertise integrative pain blocks for diabetic neuropathy. Apparently, they get weekly or sometimes bi-weekly ankle blocks, along with what they call 30-minute electronic signal treatments. They go weekly for 6-8 weeks. Some patients have come back with decent results, yet some have had no improvement. Does anyone have knowledge whether this is a viable treatment plan?

Try injecting with Sarapin. It also sounds like he could use some physical therapy, and heel lifts in his shoes. If all appropriate conservative measures fail, consider a debridement/debulking procedure with bridge anchoring after detachment.

I recommend Gerbing's™ Core Heat™ Heated Sock Liners. Although they are $200 per pair, if I were to run the race, I think that they would be worth it. I also recommend these and similar socks to my Raynaud’s patients.

I recommend Grabber Toe Warmers-  chemical packets that are thin and activated once the plastic package is opened. I use them for bicycling in the cold weather and snowboarding. These are available in Target and online.

A fee of $19/month for a patient portal is not unreasonable. A fee of $45/month for e-Prescribe is not unreasonable. First, you do not need a patient portal to achieve "Meaningful Use", though it does make your life easier, so having that as an ADD-ON with an extra fee does not concern me. Second, doctors who are not allowed to prescribe such as chiropractors do not need e-Prescribing, so having that as an ADD-ON also does not concern me. Having an EMR set up with a base fee and extra fees for these two modules is not unreasonable.
 
What does concern me is that companies should have been more up-front with you, especially about the e-Prescribing fee. Dr. Ressler really should have been made aware of the pricing policies prior to signing the contract. When shopping for an EMR, you should always get in writing a list of all features that are included and a list of all features that will cost 'extra.' Have this sheet attached to your contract. In addition, make the EMR company put into writing that every feature that YOU need to achieve the incentive is included in the price you are paying. These contracts can be long, detailed documents and there is often a significant commitment on your part.  It is always a good idea to have your attorney review the contract prior to signing the contract.

Early in my practice in Dallas in 1981, I had the good fortune to have a deaf woman come to me with a foot problem I well understood. Since I had not much else going on, I had the time to write notes to/about her that I kept as records. Her questions were also written. That patient sent me dozens of paying patients, some of whom were deaf. But they all brought their speaking family member with them as interpreters. The deaf were the most happy and healthy patients I had.
