While Cycling, UK Podiatrist is Victim of Hit and Run Driver
Police are hunting a hit and run driver who deliberately mowed down a father of two out riding his bicycle. Christian Ashby, 36, was on his regular early-morning ride on Friday when a car veered on to the wrong side of the road and hit him head-on.

Some Foot Surgery is Done to Prevent Bigger Problems Later: CA Podiatrist
“Today, a lot of people participate in sports and fitness activities that can lead to wear and tear as well as injuries,” says Scott Smith, DPM (doctor of podiatric medicine) at Monterey Spine and Joint. He also provides care at CHOMP’s wound care center treating foot wounds to prevent amputations. It’s important not to ignore pain or things that don’t look right. They just might indicate a serious condition that can lead to more severe problems in the feet as well as affect overall health.”
|
|
Dr. Scott Smith |
As Dr. Smith explains, some procedures to treat fairly common maladies like bunions are done to circumvent even bigger problems down the road. “A bunion affects the joint and can lead to arthritis,” Dr. Smith says. “Surgery isn’t always the answer, so people shouldn’t shy away from seeing a podiatrist about a bunion or other foot problem. Treatment depends on how severe it is, how much the joint and cartilage are affected, and the activity and needs of the patient.”

Ready or Not, e-Prescribing Coming to New York Providers
Physicians in the state of New York say they're mostly ready to begin prescribing drugs solely through electronic methods this week after being given an extra year to prepare. They'll be the first in the nation to face penalties if they fail to do so. Enforcement will likely be tame in the beginning, as long as physicians can show they're trying to comply, said Dr. Joseph Maldonado Jr., president of the Medical Society of the State of New York. Maldonado said the extra year has helped, though some providers have been granted waivers.
Dr. Howard Zucker, the state's health commissioner, recently announced 12 circumstances in which a provider would be exempt from the mandate. Those include prescribing products with complex directions or compounded drugs, which would be difficult to enter into an electronic system. Zucker also acknowledged that e-prescribing isn't ideal for non-patient specific prescriptions and that nursing homes likely won't be ready for the mandate, for economic and technological reasons.

CMS Tests Indication-Based Pricing With New Part B Payment Model
In a new proposed rule making alterations to the way CMS pays physicians for administering drugs covered by the Medicare Part B program, CMS on March 8 indicated its intent to implement value-based pricing tools like those used by commercial health plans and Medicare Part D plans to improve patient care and manage drug spending. One of these tools is indication-based pricing, in which CMS would pay an adjusted price based on the product’s safety and cost-effectiveness for a specific indication.
According to the proposed rule, Medicare Program; Part B Drug Payment Model (81 Fed. Reg. 13229, March 8, 2016), the first phase of the five-year payment model would revise the current reimbursement structure of average sales price plus 6% to encourage more cost-effective prescribing. As a result, CMS proposed using a fixed percentage of 2.5% and a flat fee of $16.80 per drug per day, which would be updated annually. The model calls for the participation of all providers and suppliers in certain geographic areas that administer and bill for Part B covered drugs

| RESPONSES/COMMENTS (PM NEWS QUICK POLLS) - PART 1A | |
From: David E Samuel, DPM
Regarding class sizes, Dr Kittay is 1,000% correct. It is beyond comprehension how we as a profession allow that other schools be opened to screw the students who have committed 4+ years of their lives and spend, some in excess of 200k, only to be left high and dry with no formal post-graduate training, that will limit their ability to practice and make a real living. Already our field is so specialized and those students who finish school now look for that residency program to get their surgical training. Sadly, not everyone is a surgeon, nor will some ever be, regardless of the training.
Unlike MD and DO programs, those 'wanna be' surgeons who just don't make it can be fabulous medical non-surgical specialists. We really don't have that option. Sure, some can do sports or primary care alone, but will be somewhat limited. Please, no emails about how great non-surgical can be. I'm sure it can be okay. But overall, I'm not buying it. The incorporation into MD/DO-like training with specialty foot/ankle wound, etc. residency programs will allow the cream to rise in podiatric medicine and surgery.
For now, it is the OBLIGATION of the colleges and CPME to make sure our students ALL have positions for post-graduate training or, in my opinion, prepare for legal repercussions from in-debt students with no way to pay back their debt. More importantly, right now we OWE these hard-working students the best chance to succeed and MUST guarantee post-graduate programs.

| RESPONSES/COMMENTS (PM NEWS QUICK POLLS) - PART 1B | |
From:Chuck Ross, DPM
I have been following the recent discussion relative to the number of students vs. the number of available residency slots and suspect that at some time this may become a horrible legal entanglement for the profession. As a member of the teaching staff at my hospital, I have residents who are in a 3-year medical residency program who rotate through my office. I have learned that MDs, too, do NOT have an ample number of residencies for ALL graduates, and medical schools are increasing the numbers of students entering with discussion about “new” medical schools due to the lack of primary care and internal medicine physicians.
There are also limited numbers of specialty residencies. The main difference is that, as was pointed out earlier, a podiatry graduate has no where to go without the residency, while an MD graduate has many more options. I have no idea where the prevailing winds might blow if a class action suit were initiated, but I would hate to see that much time and money involved in our defense. Solutions are not easy, but do need to be reviewed in an attempt to find an amicable resolution. This could be a bad rap for encouraging future students to seek out such a wonderful fulfilling profession.
