NYCPM Students Volunteer at Mt. Sinai Health Fair

Recently, three third-year NYCPM students volunteered at the Mount Sinai Community Health Fair. This health fair provides about 20 different health screenings, including foot health, free of charge to the East Harlem community. NYCPM students, Michelle Kaufman, Amanda Riley, and Naeemah Ruffin (all 2016) joined with three first-year medical students from the Icahn School of Medicine at Mount Sinai to conduct the foot screenings. Ruffin trained the first-year medical student volunteers to properly conduct a foot screening exam.

TX Podiatrist Discusses Use of Implants for Hammertoe Surgery 
 
Many women love high-heeled shoes but an embarrassing foot condition can keep them off their feet. One doctor says a new procedure can correct a painful problem called hammertoes. Doctors say toe pads, cortisone shots, and anti-inflammatory drugs can bring some relief, but many end up turning to surgical correction. Traditionally, wire pins are inserted through the bones. "And the pin would stick out of the top, and then we'd pull these pins out after surgery. Very medieval,” podiatrist Dr. Robert Moore said.

A new procedure uses an implant rather than those metal wires to permanently straighten the joint. "These are made out of cortical bone, and actually stay in the bone marrow, and keep the toes straight but being non-metallic will eventually incorporate in the bone and will not have to be removed,” Moore said.

IN Podiatrist's Forensic Testimony Helps Convict Murderer
 
A 25-year-old man from Newport News, VA will serve 35 years in prison for a fatal shooting in 2012. A Circuit Court judge on Wednesday sentenced Shymeek Stanfield to 60 years in prison, with 25 suspended, said Commonwealth's Attorney Georgette C. Phillips. A shoe was left at the murder scene, and detectives drew on forensic podiatry to assist their investigation in Stanfield's case. Forensic podiatrists look at bare footprints and footwear to see if they can identify who wore a shoe.

Dr. Michael Nirenberg, a podiatrist from Indiana who also does forensic exams, read about the case and contacted Smithfield police to offer his help. Investigators sent Nirenberg photos of the inside of the size 11 Nike tennis shoes that Stanfield was wearing when he was arrested following the direct indictment. Nirenberg's report was introduced as evidence at trial, and the doctor also testified.

The YWCA Darien/Norwalk has announced the six honorees for the sixth annual Women of Distinction Awards and Luncheon on May 7. The distinguished honorees are Sandy Nielsen Baumann, Alicia Sillars, Dr. Josephine Velazquez Rucquoi, Jane Yezzi, and Susan Delaney.

Dr. Rucquoi, along with dentist Dr. Maryann Lehmann and two other doctors, founded Doctors4Darien, a group that educates the community about healthy living through local forums. She also runs road races as part of promoting a healthy lifestyle. She has contributed important information to presentations at The Depot, and acts as a role model for young women, working as an Elder in the Hope Church with teens and singles.

NY-based Start-up Uses 3-D Printers to Create Custom Insoles or Orthotics 
 
The world of apparel is moving in two new directions. More of the clothes we wear are destined to be better customized for our bodies thanks to the ubiquity of smartphones that can double up as scanners. They’re also becoming smarter, with startups and researchers looking at ways to integrate circuits and sensors seamlessly into fabric. One company that’s working on doing both is SOLS. The New York-based startup uses 3-D printers to create custom insoles or orthotics that its customers can wear in their shoes.

Query: Walking Boot & Night Splint Coding
 
Has anyone heard about the new codes for walking boots and night splints? 
 
Instead of L4360 and L4396, it they would be L4361 and L4397, respectively, if you use the kind of devices that do not require substantial modifications. Any additional information would be appreciated.

Response: I had a meeting with our DME contractor about this very issue. The coding selection has to do with how much modifications are required. The key is whether "significant modification" was performed. The fitting and maybe adding a wedge or an off-loading accommodation does not seem to be enough to validate the term "significant". Dr. Kesselman, I believe, in previous posts on Codingline explains some of the expectations regarding the new and old codes. 
 
The good news is that there is no reimbursement differences between the two codes at this time. That may change though in the future.

I have used the "football dressing" for many years, but usually I use an Unna boot first, then the Webril, then Kling, and then Coban. It is a soft version of a full contact cast, and it is effective.

Tenex has also been determined by major Blue Shield companies as being experimental and investigational. The probe is a class-one device with NO long-term follow-ups. I have also been told that the billing codes used for the Tenex procedure are not correct. It isn't a tenotomy procedure as listed by the Tenex company. It could be the old insurance saying... "do the procedure, bill the procedure, get paid for the procedure, and then get audited and pay back the monies."

3) Are most patients obtaining their desired long-term results in one treatment?

These boots are inspired by moonboots, cosmic objects, and space in general. The main concept is having the moon boot without heel, because there is much less gravity on the moon than there is on the earth. That's why I made the sole of the shoe flat with a square-shaped sole profile to help friction attach a grip on the ground.
