PODIATRISTS AND DIABETES

Team Approach Necessary to Prevent Diabetic Ulcers: CA Podiatrist

If untreated, a blister or small cut can become a diabetic foot ulcer (DFU), which is the biggest risk factor for amputation,” explains Lee C. Rogers, DPM, co-director of the amputation prevention center at Valley Presbyterian Hospital in Los Angeles. Daily foot care and glucose control are essential, but proper foot care requires teamwork, especially once you have a wound.

Dr. Lee Rogers

According to Dr. Rogers, no single medical specialty is capable of providing the care necessary to prevent and manage foot ulcers. Regular visits to a podiatrist are essential. But once you have a DFU, the team will consist of, at minimum, a vascular surgeon and a diabetic podiatrist, but may well require an infectious disease specialist, a wound specialist, and others as well.

Indian Podiatrist Discusses How to Fight Fungal Infections

Dr. Govind Bisht Senior Consultant, Podiatry with Delhi-based Sri Balaji Action Medical Institute gives you an easy tip to fight fungal infections, "Before going to sleep, apply moisturizer containing 7% to 10% of urea and paraffin on your feet. This will help you in getting rid of the dead skin (keratolytic effect) while keeping the feet clean, smooth, and allergy-free.

Dr. Govind Bisht

"Avoid putting moisturizer between the toes as web spaces have moist skin and putting moisturizer will increase dampness and may invite fungal infection," says Bisht.

PODIATRISTS AND FOOTWEAR

Non-Supportive Flip-Flops are Worst Shoe Offenders: NY Podiatrist

The best shoes are those that follow the natural arch or form of your feet. Podiatrist Dr. Andrew Shapiro revealed that the worst offenders are flip-flops because they don’t provide sufficient and appropriate arch support, thus not protecting the foot at all. This could lead to plantar fasciitis, broken toes, and scraped heels.

Dr. Andrew Shapiro

If you are forced to wear them, wear them only for a short time and where it is appropriate. You could use flip-flops with a back strap to help anchor the flip-flops better.

RESPONSES/COMMENTS (CLINICAL) - PART 1A

RE: Freiberg's Infraction in 47 Year Old Male
From: Charles Morelli, DPM

Having ruled out neoplasm or any other contraindication, consider inserting an Arthrosurface implant to remodel and resurface the metatarsal head. They work well on the first metatarsal. I have used them about five times for lesser metatarsals for both Freiberg's and for trauma cases. Motion can be limited after this procedure, but patients' symptoms are usually addressed successfully.

RESPONSES/COMMENTS (CLINICAL) - PART 1B

RE: Freiberg's Infraction in 47 Year Old Male
From: Sloan Gordon, DPM

I suggest you rely on the bone biopsy at the time of surgery, since the MRI will not alter your treatment plan.

Titanium Cap

As for treatment, I have been successful with resecting the infarction and replacing it with a titanium cap.

RE: Scared Heart Hospital Closes (Darrell Latva, DPM)
From: Greg Amarantos, DPM

Yes, Scared Heart Hospital has closed. Swedish Covenant Hospital in Chicago, under the direction of Dr. Ted Polizos and me, as co-directors of a new residency program, has been in contact with CPME and is in the process of accepting the 6 residents from Sacred Heart as an emergency transfer.

RESPONSES/COMMENTS (CODINGLINE CORNER)

RE: E/M with Nail Debridement Codes
From: Marc Katz, DPM

I wanted to help clarify the use of E&M codes with routine foot care. Podiatrists have routinely been in the habit of automatically billing an E&M code whenever they rendered routine foot care. Under the current Medicare rules that would be considered fraud, and the OIG is currently investigating podiatrists that make this a practice.

Some of those people end up as the headliners on PM News! These podiatrists also ruin it for everyone else who...

RESPONSES/COMMENTS (RESIDENCY SHORTAGE CRISIS)

RE: Unmatched Residency Placements Currently Stand at 86
From: Jon Purdy, DPM

With all of the fundraising that is going on for students and other podiatric campaigns, why have I not seen one for this “crisis”? It seems to me that if there are more students than there are residency spots, there should be something named “The Residency Education Fund”. Why are the schools and the APMA not working diligently with the CPME in the formation of a campaign to encourage podiatrists to become residency directors?

Why don’t these entities have a “program” whereby they have dedicated people able to match hospitals with potential residency directors and facilitate the...
