PA Podiatrist Discusses Platelet Therapy

Achilles tendonopathy and plantar fasciitis--big terms for two medical problems that can make every step hurt. Medications and therapy often  help, but if they don't give you relief, surgery's usually your next option. Now, a doctor in our region offers a new choice.

It involves taking platelets from a patient's own blood and injecting into the source of the pain, inflammation in the foot. At Blair Orthopedic Associates, Foot and Ankle Surgeon Dr. Adam Budny explains, "we use a centrifuge, and that spins the blood down, which concentrates the platelets and we re-administer it to an injury." The platelets contain natural substances in the body that promote healing of the inflamed tendon.

Two Tests for Posterior Tibial Dysfunction

How can you tell if your posterior tibial tendon is dysfunctional? There are two good ways: "Too Many Toes Sign" tests for flat-foot deformity or over-pronation, and "Jack's Test" determines if your posterior tibial tendon can correct this abnormal position. If one or both of these tests are positive, your tendon is at risk, and you may have PTTD.

To perform "Too Many Toes Sign," stand facing away from someone and have them draw an imaginary line from the center of your calf to the floor. If your heel lies outside this line and more than two of your toes can be observed, the test is positive.

To perform "Jack's Test," lift your good leg off the floor, rise up on the toes of your bad leg, and hold that position. A diagnosis of PTTD may be confirmed if you are unable to perform this one-legged heel-raise or if your heel remains turned outward in the tiptoe position.

Podiatrist Tells Kate Middleton to Fix Her Feet

Kate Middleton is barely seen out in public engagements without her trusty nude LK Bennett high heels on. When she made a pre-Olympic visit to a judo center however, she gamely kicked off her heels to step on the mat. She may be regretting that decision now as a podiatrist has taken it upon herself to publicly point out every single flaw of her feet. In an open letter to the duchess published in the Daily Mail, podiatrist Emma Supple pointed out what's wrong with her feet already and warned her to take it easy with the heels.

The podiatrist proceeds to describe in detail what's wrong with her feet. From the yellowing of her left foot's little toenail to her developing bunions, which are at a very early stage, nothing goes unnoticed. By writing such a public letter, podiatrist Emma Supple may get more business from the British public for her practice, but she'll probably never treat Kate Middleton.

NY Governor Signs Expanded Scope of Practice Law

NYSPMA President, Gary Stones, DPM and Executive Director Len Thaler have congratulated Governor Cuomo for signing A9293A, a measure that will expand the scope of practice to include essential ankle and foot care services by podiatrists. Until now, New York was one of six states that significantly limit the practice of podiatry. The law will require some slight modifications and will go into effect within 18 months.

"We are pleased that New York will now join 44 states in refining the definition of podiatry in order to reflect the medical education relating to the treatment of malfunctions in the foot. This refinement will improve patients' access to a fuller array of healthcare services and practitioners while assuring a greater quality of care," said Stones.

The “American Board of Podiatric Medicine” is Officially Introduced

On August 17, 2012 the American Board of Podiatric Orthopedics and Primary Podiatric Medicine became the American Board of Podiatric Medicine (ABPM). The name modification coincided with the Board’s 20th anniversary and was officially rolled out to the ABPM membership at its Annual Meeting of Members.

The decision to “do business as” the American Board of Podiatric Medicine was a result of several years of Board planning and significant input from a survey of its Diplomates who requested the change. The new name  covers  all the clinical areas represented by this Board’s certification. In addition, the name modification represents the evolution of podiatric residency training and current podiatric practice.

The International Foot & Ankle Foundation for Education and Research has just received a very generous donation to the IFAF Residency Education Fund from one of America's Prominent Podiatric Physicians, Dr. David C. Novicki, of Milford, CT.

Dr. Novicki, a noted international author and lecturer and frequent speaker at IFAF  seminars, graduated from Temple University School of Podiatric Medicine. He has served as past president of the American College of Foot and Ankle Surgeons, the American Board of Podiatric Surgery, and the Connecticut Podiatric Medical Association. He is currently a Clinical Assistant Professor of Orthopedics, Yale University School of Medicine and Chief, Section of Podiatric Surgery at Milford Hospital.

Podiatry Clinic Cancellations Prompt Peninsula Trust Apology

About 140 patients requiring treatment on their feet and legs had appointments cancelled at short notice. Peninsula Community Health (PCH) said "unexpected" staff sickness had forced it to suspend the clinics, although that had now been addressed.

But Diabetes UK said missing routine treatment could lead to complications which could result in amputations. A podiatrist diagnoses and treats conditions which affect the foot, ankle, and lower legs. These range from relatively minor conditions, such as corns and bunions, to serious issues such as ulcers and circulatory disorders.

FL Podiatrist Named Osteopathic Medical School Professor of the Year

Leonard A. Levy, DPM, MPH, was named the 2011-2012 Nova Southwestern University (NSU) College of Osteopathic Medicine Professor of the Year by the university provost. This is the first time a podiatric physician was named for such an award by a U.S. medical school. It is conferred on a full-time faculty member serving the university at least ten years, who has achieved national and international prominence, has earned a distinguished reputation by significant contributions to research, scholarship, and exceptional instruction.

Medicare Website Redesign Complete

A redesign of the medicare.gov website is now complete, making content more accessible and easier for beneficiaries, their families, and caregivers to understand.  Today’s redesign, announced by Centers for Medicare & Medicaid Services (CMS) Acting Administrator Marilyn Tavenner, supports CMS’s commitment to provide better customer service.

The new site will allow most users to find the content they’re looking for directly from the home page. These features include:

Recipients of Drug-Company Payments Drawing Attention of Federal Fraud Enforcers

It takes two sides to create a kickback. But until recently, the recipients of illegal payments in healthcare have faced far fewer legal consequences than the huge corporations accused of handing out the forbidden lucre. Many possible reasons have been offered as to why physicians and other healthcare providers have historically faced little, if any, scrutiny in cases of alleged kickbacks and bribery and subtler forms of manipulation of drug-prescribing patterns. But that may be changing.

In 2008, HHS' inspector general's office started including requirements in corporate integrity agreements with large drug companies that any and all payments to doctors be publicly disclosed on their websites. Two years later, a similar requirement was enshrined industrywide in the Patient Protection and Affordable Care Act. Known as the Physician Payments Sunshine Act, the reform provision kicks in March 31, 2013, and will require all drug companies and devicemakers to disclose payments to doctors. Those disclosures will be published online annually, according to the law.

Steps to Writing Faster

Create high-quality content quickly and efficiently by taking the time to organize your thoughts and establish a plan. Work in small increments to stay focused and avoid including irrelevant material. Follow these steps:
1. Begin with the headline. Identify a problem that truly interests your target audience. Choose words for the headline that grab attention and reveal your point of view.
2. Write the hook. Use several sentences or paragraphs to explain the symptoms of the problem and describe the difficulties that the problem causes. End by declaring that you have a solution.
3. Outline the main sections. Break your topic into separate sections that list the problem, discuss the causes, outline the solution and describe how readers can implement the solution. Come up with section headings and add brief notes to each section.
4. Fill in the blanks. Write one section at a time by expanding on your notes. Remember to cover what you promised in the headline, hook, and section headings.

Letting go of any TEAM member, especially one who holds a managerial position is difficult. The most important thing to remember is that your practice is your business and your life line. As time goes on and we develop relationships with our TEAM, it becomes a matter of looking at the impact every individual has on your practice. It is wonderful to have nice people working with you, but "nice" doesn't mean efficient, and it certainly doesn't pay the bills.

My suggestion would be to  re-assign your manager while training someone new to take her place. Sometimes, a different title will change the way a person performs and will affect the entire dynamic of your office. I highly suggest making sure that you can afford this title change as well as a new, possibly part-time manager. Be respectful of your current manager's feelings, but ultimately think of the future of your practice.

Research - The primary purpose of this fellowship is to provide the Podiatric Surgeon who has completed a minimum of a three year residency, and who is committed to a part-time/full-time academic career in Podiatry, further education on research of the Diabetic Foot. I moderated two sessions on how to counsel obese patients. One was at a prior APMA national convention and the other at a SAM regional conference in Orlando. At both of these seminars, we had nationally respected authors on nutrition, behavoral modification as well as surgeons who are experts in the area of gastric bypass and band procedures. Attendees left these sessions confident on how to advise patients and make proper and timely referrals to patients whose foot problems were being caused or made worse by being overweight.

Although those sessions are long gone, it behooves all of us to seek out sources for behavoral modification and/or surgical options available to our patients in our individual localities and make these timely referrals when necessary.

Of course, it is within the scope of our practice as podiatric physicians to discuss whether a patient is obese or not. Excessive adipose tissue results, with each step, more compression force on the plantar heels and plantar forefoot, and more tension force within the supporting ligaments and tendons of the foot which may lead to mechanically-based pathologies of the feet and lower extremities. On a daily basis in my practice, I counsel patients that that their obesity may be one of the causes of their foot and/or lower extremity pathology, and that they would help themselves by starting on a weight loss program. I believe that this is one of my responsibilities as part of the healthcare team who are providing medical care to the individuals of my community.

However, it must be remembered that the body mass index (BMI) cannot be used as a reliable indicator of percent of body fat in all individuals since BMI underestimates body fat in elderly individuals, overestimates body fat for Polynesions, underestimates body fat for Asian-Indians, overestimates body fat in athletes and military personnel, overestimates body fat in individuals undergoing resistance exercise training and can overestimate body fat in patients with water retention (Prentice AM, Jebb SA:  Beyond body mass index. (Obesity Reviews, 2:1-7, 2001). Therefore, the clinician who uses only BMI to give healthcare advice to patients needs to understand that BMI is not a reliable or valid indicator of body fat percentage in many individuals.

Having just returned from a few weeks in London and northern Europe, I can tell you that the fascination about each and everything that princess Kate does is incredible. Imagine the press of Madonna, Lady Gaga, Tom Cruise, and Charlie Sheen all put together around one person. I saw stories about Kate’s hats, shoes, and the colors of her clothes.

I commend Ms. Supple, for seizing an opportunity to point out items that need to be addressed with her feet before the future queen of England gets her crown. While this seems like a trivial matter, I can remember that when President Reagan received a colonoscopy, this seemed to change insurance coverage and the profession of proctology to become colon rectal surgeons. The Brits love their royalty and, who knows, Kate getting her feet treated, could be an event that changes podiatry in the UK.
