LA Podiatrist Discusses Bunion Surgery

It is hard for avid runner, Lisa Morgan, to be out of commission with her feet. This Lake Charles wife and mother of teen twins is a few months post-surgery for a bunion on her right foot and just a few weeks away for her left. The big toe's shift causes problems for the second toe. "The second toe could either lift up, it can underlap, it can hammer up," said podiatrist Dr. Tyson Green.

Dr. Tyson Green

Treatment in Lisa's case was a bunionectomy surgery, using a plate with locking screws for stability. "We're actually fusing the joint in the back that holds this alignment in place, and we don't have the chance of it coming back over and recurring," said Dr. Green. If the deformity is not as severe, a tightrope procedure can be done by weaving a suture through the first two toes. "We are able to have a little bracket so that when we tie it down, it pulls that metatarsal bone back into place," said Dr. Green.

INTERNATIONAL PODIATRISTS IN THE NEWS

Training for Ballet Competition Results in More Foot Injuries: Aussie Podiatrist

Mackay podiatrist Jemma Riddiford said she has noticed a dramatic increase in dancer injuries in the lead-up to the ballet competition known as the Mackay Eisteddfod. "We're seeing tendonitis, foot pain, and complaints about ingrown toenails," she said.

"A lot of kids are getting on pointe a little bit too soon. The rule of thumb is that little kids shouldn't be getting on to pointe before they're 12.  They shouldn't be spending more than one hour a week on pointe."

Providing Free Preventive Care Can Save the Nation Billions: CA Podiatrist

At Care Harbor, a free 4-day clinic, Podiatrist Dr. Franklin Kase has diagnosed new patients with diabetes and high blood pressure with one check of the feet. "A lot of these people, interestingly enough have jobs, but don't have the health insurance to access medical care," says Kase.

Dr. Franklin Kase

Dr. Kase says that this preventive care helps all of us because if there are no treatments of diseases such as diabetes, these patients wind up in emergency rooms, which costs the nation between one and two billion dollars a year.

PRACTICE MANAGEMENT TIP OF THE DAY

Improve Your Phone Technique

Connect better with people over the telephone by projecting a caring and enthusiastic image. Adopt these habits:

  •  Check first. Before you say too much, ask “Is this a good time for you?” Be willing to call back later if you can have the person’s full attention then.
  •  Block distractions. Turn away from your computer or the open doorway, or close your eyes so you can focus only on what the person is saying.
  •  Add energy. Stand up when you talk and gesture as you would if you were having a conversation in person. Your enthusiasm will come through.
  •  Invite comments. Ask open-ended questions, and use the word “You” more than “I.”
  •  Smile. It does make a difference in your voice tone, even if the other person can’t see you.

CODINGLINE CORNER

Query: Post-op Casting

Can you bill for the cast materials and application during the post-operative period? I recently received a denial of payment for patient treatment included in the global fee. Should this be appealed?

Marc Garfield, DPM, Williamsburg, VA

Response: Yes, appeal. You may bill for cast and materials applied during the global period. Simply resubmit the claim with a "-58" modifier appended to the application code.

RESPONSES / COMMENTS (EMR)

RE: EMR Stimulus Incentive
From: Frank Lattarulo, DPM

After the long and arduous task of converting to EMR and meeting meaningful use, I am happy to report that almost exactly 5 weeks after submitting, I received my meaningful use reimbursement of $18,000. I encourage all of you to move forward with EMR as it is well worth the investment from a patient care standpoint as well as a financial one.

RE: Online Software for Appointments (Andy Karpf, DPM)
From: Larry Kosova, DPM

I have used Eppointments Plus since 2003. You can use it as a stand-alone product or have it integrated to your EMR. You can make it 100% custom to your practice, and it's very easy to use with excellent customer service. You can use your smartphone which helps checking your schedule before you leave for the office.

Patients can make appointments, and they can receive reminders and thank-yous about their appointments. There are a whole host of features that I am not even touching upon, but I and my patients have been very happy with the product.

RE: Sigma Ortho Digital X-Ray System (John Moglia, DPM)
From: Larry Kosova, DPM

I have had the Sigma system for about 4 years now, and I am very happy with the product. I have their original scanner and plates, but I know there is a new system now. Their customer service is top notch and can fix most issues through remote connection. The unit is very fast and efficient and easy to use. They installed it in our office at noon, and we were using it by the first 1:00 pm patient. The images are great, and patients seem impressed with what we are able to do with it and love the convenience. Once you start with digital x-rays, you will wonder why you did not do it sooner.

RESPONSES / COMMENTS (NEWS STORIES)

RE: International Podiatrists in the News
From: Robert Bijak, DPM

The beginning of PM News generally starts with news articles about podiatrists. Lately, foreign "podiatrists" have entered into some news articles and I'm concerned about the "image" of U.S. podiatry being considered in the same sphere as foreign "misters" who, in many respects, have only a bachelors degree in "podiatry" and are not considered doctors in their country.

Some of the inane comments on foot health have come from both sides of the ocean (flip-flops, high-heels, orthotics, shoe fitting advice, asthetic podiatry), but the French "podiatrist" stating that the foot is the most complicated part of the body should strain the credulity of the most vampant podiatric supporters as hyperbolic.

I do not believe we do ourselves good by calling our European friends "podiatrists" generically. Though not as good as it should be, I believe our training and practice is at a different level than that of the Brits, Aussies, French, etc.
