IN Podiatrist Collaborates with Indian Researchers
An Indiana podiatrist’s scientific collaboration with Indian researchers has led to the publication of their findings recently in the Journal of Forensic and Legal Medicine. Dr. Michael Nirenberg, a podiatrist at Friendly Foot Care in Crown Point, teamed with Dr. Kewal Krishan of Panjab University and Dr. Tanuj Kanchan of the All India Institute of Medical Sciences in Jodhpur.

The task of identification based on the analysis of footprints can be challenging when the footprints belong to one of two identical twins. The study looked at the quantitative measures of the foot impressions in the footwear of adult identical twins. The observations derived from the study are likely to assist forensic investigations in cases involving the foot impressions of twins.

Once Illiterate Aboriginal Aussie Becomes Podiatrist Lecturer
An Aboriginal man who was once illiterate has risen to become a Doctor of Podiatry who writes international research papers. Dr. James Charles began his incredible journey after his young sons brought home books from pre-school and he could not read them. His secret of illiteracy became too much to bear and he vowed to overcome his lack of reading and writing skills. What followed was a journey through adult learning centers and five universities.

Charles, 49, is now a Lecturer in Podiatry at Charles Sturt University. He hopes his story will inspire others. “I am continuing to read and learn every day and am writing papers and publishing.” He has had six papers published in international journals and has another half-a-dozen in the pipeline.

Judge Acquits OH Podiatrist of Sexual Assault Charges
A Westlake podiatrist accused of sexually assaulting a 23-year-old medical assistant was found not guilty on all counts Thursday afternoon. Anthony Polito hugged his lawyers and family members and cried in the courtroom after Common Pleas Judge Shannon Gallagher said there was not enough evidence to prove Polito assaulted the woman.

Gabapentin Linked to Increased Risk of Opioid-related Death
Concomitant use of gabapentin and opioids is associated with a significantly increased risk of opioid-related death, according to a Canadian case-control study. "When we started this study, we were aware of the potential for an interaction between opioids and gabapentin but weren’t sure whether the combined use of these drugs led to clinically relevant risks,” Dr. Tara Gomes from St. Michael's Hospital and University of Toronto, in Canada, told Reuters Health by email. “Therefore, our finding that even moderate doses of gabapentin were associated with a 60% increased risk of opioid-related death was important, and the fact that this increases to a twofold higher risk among people getting very high doses is concerning.”
The odds of an opioid-related death were 49% higher among people recently exposed to gabapentin and opioids, compared with those exposed to opioids alone, even after adjustment for a variety of potential confounders. Moderate daily doses of gabapentin (900 to 1799 mg) were associated with 56% increased odds of opioid-related death, high doses (800 mg or more) with 58% increased odds, and very high doses (2500 mg or more) with 83% increased odds. In an exploratory analysis, 46% of gabapentin users in 2013 received at least one concomitant prescription for an opioid.

MedPAC Urges Repealing MIPS
The Medicare Payment Advisory Commission (MedPAC) is pushing for the immediate repeal and replacement of a Medicare payment system that aims to improve the quality of patient care. To avoid penalties under MACRA, physicians must follow one of two payment tracks: the Merit-based Incentive Payment System, or MIPS, or advanced alternative payment models like accountable care organizations.
MedPAC wants to junk MIPS as it feels that it's too much of a burden for physicians and won't push them to truly improve care. "Time is of the essence to develop an alternative for MIPS," said David Glass, principal policy analyst at MedPAC. It "will not achieve the goal of identifying and rewarding high-value clinicians."

Things to Consider When Retiring from Practice
1. Make the transition "before you have to."
One physician summed it up this way: "I would never want to put one of my partners in the position of saying it was time for me to go because of clinical reasons. Imagine the personal anguish that would cause them. And, what a personal blow to receive that feedback."
I suggest that doctors think realistically about how long they plan to practice. This is a very personal decision and it's different for everyone. Although none of us like to admit it, physical and mental stamina decrease with age. It's important that physicians be attentive to the signals that indicate they should begin limiting their procedures and cases before the transition out of practice.
