PODIATRISTS IN THE NEWS
IN Podiatrist Discusses Ingrown Toenails

Ingrown toenails happen when the toenail grows down instead of out, leading it to dig into the surrounding skin. Hence why they’re normally very uncomfortable. This most commonly happens in the big toes, and can be caused by anything that leaves the nail deformed. “Common causes include ill-fitting shoes where the toes are cramped within the shoe, trauma to the nail (i.e., a fallen object or collision/impact) causing nail deformity, and nail fungus resulting in thickened nails,” Quinton Yeldell, DPM, founder of foot care brand Southern Hospitality, tells SELF

Dr. Quinton Yeldell

Your foot type can also make you more prone to ingrowns. Flat feet can make it harder to find shoes with the right fit, and repeatedly wearing shoes that don’t fit and leave your toes banging against the front can lead to ingrown nails, Yeldell says. Other conditions, like bunions, can also increase the amount of trauma your poor little toes endure. “Ingrown nails that show signs of infection (pain, swelling, redness, and discharge) should be treated by a podiatrist immediately,” says Yeldell. Messing with it on your own will only make matters worse. If you’re questioning whether or not you can handle it on your own, don’t. Let a podiatrist take care of it.

PODIATRISTS AND POLITICS
OH Podiatrist is Running for County Commissioner

Dr. Ted Bowlus

Along with presidential and U.S. senate races on the ballot, northwest Ohio voters will decide many county offices. Wood County residents will decide whether to retain two sitting county commissioners or replace them with new faces. Democrat incumbent Joel Kuhlman is seeking his second full term. He is being challenged by Republican Dr. Ted Bowlus, 67, of Pemberville, who is a podiatrist and former president of the Wood County Health Board.

PODIATRISTS IN THE COMMUNITY - PART 1
MN Podiatrist Hosts "Cast Wall of Fame"

Dr. Loring Stead's definition of a casting call may differ from the general population, but it has often endeared him to his youngest patients. The veteran podiatrist at Olmsted Medical Center has been collecting his favorite casts throughout his 30-plus year career. His favorites are currently displayed on a Cast Wall of Fame in the room next to his office, periodically changing as creative colorings are cut off.

Dr. Loring Stead (photo: Ken Klotzback)

The quirky cast collection was created in 1989 when Stead's daughter, Danielle, then a pre-schooler, saw a young patient sporting an elaborate rending of "The Little Mermaid" on her cast. Obsessed with Ariel and Flounder, Danielle urged her father to keep the work of art rather than throwing it out. Nearly 30 years later, the original cast continues to headline the display.

PODIATRISTS IN THE COMMUNITY - PART 2
Annual Memorial 5K Walk/Run to be held for PA Podiatrist

Friends of Dr. Gina Carlo will host an annual 5K walk/run commemorating Dr. Carlo’s “all too short but very inspiring life,” according to Nancy Osborne. The event in memory of the podiatrist from Clarks Summit, who had an office in Carbondale, is scheduled for Saturday, Nov. 5, at the Carbondale Area Junior/Senior High School football field and track.

Dr. Gina Marie Carlo

Three area animal shelters received donations, and Carbondale Area graduating seniors Conor Durkin and Aislinn Toolan each received $2,000 scholarships from proceeds from last year’s event to continue their education in a healthcare field. “Our goal is to continue this tradition in the hopes of securing a solid future for people like Gina to continue the good works that she started while she was here with us,” Ms. Osborne said.

PRACTICE MANAGEMENT TIP OF THE DAY
6 Ways to End a Doctor-Patient Relationship - Part 1

As doctors, we have all treated patients who were very difficult to deal with, due to their non-compliance, an abusive attitude, or just a mismatch of personalities. For most of us, there is no need to stay in a relationship that is clearly not serving the patient nor the doctor well. What is the best way to terminate a doctor-patient relationship?

1)  Documentation is key. We must document all those behaviors and interactions that led up to this decision. If a patient abuses your receptionist, document it. Make it clear to anyone who may read your chart later on as to why this decision is being made.

CODINGLINE CORNER
Query: ICD-10 Metatarsal Coding

With the new ICD-10 code for metatarsal varus (Q66.22 [congenital metatarsus adductus; Inclusion Term: congenital metatarsus varus]), which of the metatarsal codes would be most appropriate for a prominent metatarsal head? M77.41 (Metatarsalgia, right foot), or M21.6x1 (Other acquired deformities of right foot), or Q66.22?

Edward Stein, DPM, Peters, MO

Response: The answer to your question starts with, is this "acquired" vs "congenital"? A prominent metatarsal head is a deformity which is usually acquired. The most specific codes for an acquired deformity are:

1) Other acquired deformities of right foot M21.6X1
2) Other acquired deformities of left foot M21.6X2

Q66.22 is "congenital metatarsus adductus or metatarsus varus". This is defined as a foot deformity noted at birth that causes the front half of the foot, or forefoot, to turn inward.
