NYSPMA BOT Holds Strategic Planning Meeting
 
The Board of Trustees of the New York State Podiatric Medical Association (NYSPMA) held their annual strategic planning weekend in Buffalo on October 4-5th followed by a board meeting on Sunday, October 6th. Both meetings focused on public and population health, membership and legislative initiatives. The meetings were also attended by association staff members, Dan Kline, Allison Hogan, and Sonia Lunn.

BCHC Names Director of Podiatry and Welcomes New Podiatrist 
 
Noahleen Betts, DPM, has been named Director of Podiatry at Berks Community Health Center (BCHC). Previously in private practice since 2008, Dr. Betts joined Berks Community Health Center in 2015. A graduate of the New York College of Podiatric Medicine, she completed residency training at Roxborough Memorial Hospital which included forefoot surgery, rearfoot reconstructive surgery, and comprehensive wound care.

Podiatrist Zain Chaudry, DPM, has also joined the BCHC provider staff. Dr. Chaudhry provides foot care to patients at the Rockland Street health center location. A graduate of the University of the Sciences in Philadelphia, where he received a Doctor of Pharmacy degree, Dr. Chaudry then earned a Doctor of Podiatric Medicine degree from the Temple University School of Podiatric Medicine. He completed his residency at the Philadelphia Veterans Affairs Medical Center.

How to Have a Well-run Practice - Part 11
 
The daily grind of seeing patients can sometimes be, well, grinding. It’s important to take a step back and reconsider how you do things. There’s likely another way to achieve the same, if not better, result, especially when it comes to staffing. Here’s a sample of what 943 practice owners, managers and physicians say has helped them create a better environment.
 
11. We test extensively, including basic skills as well as personality testing, plus a 1- to 1.5-hour interview. We make it clear we respect our staff, and we protect them (mainly from patient/rep. harassment). We train them intensively and, when the time comes, help them find their next step up professionally. This inculcates extreme loyalty. As a result, our turnover is very low.

It would be imperative to have lateral weight-bearing and axial sesamoid views to really evaluate this condition. In my opinion, it sounds like the patient has pain unrelated to findings seen on the x-ray provided. The arrow is pinpointing the dorsal medial cutaneous where nerve damage or entrapment could be the culprit. With dorsal medial approach bunion incisions, the incidence of damage to the DMC nerve is 50%. 
 
Palpate the exact area of pain and inject .5 cc lidocaine. If the condition improves, the patient may require removal of the distal portion of the dorsal medial cutaneous nerve and burying the distal nerve into the bone. This can rid stubborn pain in this area. Even if there is continued radiological deformity, the pain will go away by denervation of the area. Performing biologic injection and or removal of the sesamoids, fusion, etc. will further the pain. The joint is not pathological enough to justify fusion or injections. This is likely a soft tissue nerve issue. Reference: Dorsomedial cutaneous nerve syndrome: treatment with nerve transection and burial into bone. Miller SD, Foot and Ankle Int. (Foot Ankle Int 2001 March, 198-202).

As this discussion is taking place, I received an attestation from one the hospitals that I am on staff. I have to attest to receiving the flu shot and provide evidence. If I am not immunized, I must wear a mask in clinical areas of the hospital. This is not a new policy.

The government has a special fund called the National Vaccine Injury Compensation Program to compensate individuals who have been permanently injured by vaccines of various kinds including seasonal flu vaccines. According to their website, to date, it has paid out $4.2B in injury settlements. This does not include amounts paid out via traditional lawsuits, so who knows what the real damages are. The site says the injury compensation rate is about one person per million vaccines administered. However, I have had a fair number of patients over the years that suffered Guillain-Barré syndrome after various vaccines, so I have to wonder how accurate their data really is? 
 
I imagine most of the general public is not even aware of this fund or the potential risks of getting vaccines as I have not seen it discussed by the mainstream media, and therefore individuals may not seek damages when injured. It’s something to think about and might help your patients get compensation if they have major side-effects. The website is an eye-opener.
