CA Podiatrist Discusses Childhood Obesity
 
Our nation's children have entered a perfect storm for becoming obese - consuming diets that are high in fat, sugar, and salt, while engaging in an increasingly sedentary lifestyle, where they are tethered to computers, television, video games, tablets, and smartphones, which have replaced running, biking, swimming, slides, swings, and monkey bars, said California Podiatric Medical Association (CPMA) President Thomas J. Elardo, DPM.

Obesity affects almost every organ in the body, and its impact is even more detrimental in children in that their bodies are still growing and developing, said Dr. Elardo, a podiatric physician and surgeon in private practice in Los Gatos, California. Disease consequences of obesity include increased cardiovascular disease (high blood pressure, heart attack and stroke), increased rates of diabetes, breathing problems, including asthma and sleep disturbances - namely sleep apnea, increased digestive and liver problems, and increased orthopedic problems, including degenerative joint disease of the back, hips, knees, ankles, and feet.

TX Podiatrist Uses High Frequency Wave Machine to Treat Plantar Fasciitis 
 
Dr. Caesar Zuniga, a podiatrist with the El Paso Orthopaedic Surgery Group, said the patients he sees are very active, or on their feet most of the day. "I've had soccer players, men and women in the Armed Forces and I've had a few policemen," he said. "Anyone who sprints, runs, or jumps a lot - those are the people who have more problems with plantar fasciitis."

"The evolution for treatment of heel pain has progressed from having an open incision which is 2 to 3 centimeters and requires you to be non-weight-bearing and in a cast for four to six weeks," he said. "That evolutionized to an endoscopy procedure, which was two small holes that needed to be done arthroscopically where you have a blade that actually cuts the medial band of the plantar fascia." Using Tenex Health TX, a small instrument that vibrates high frequency waves to break up scar tissue, Zuniga now does the procedure in under 10 minutes using a local sedative.

QUERIES (CLINICAL)
Query: Varicosities as a Cause of Tarsal Tunnel Syndrome
 
According to a terrific article "Tarsal Tunnel Syndrome: Part 1" by Dr. Kyle Scholnick [Podiatry Management, January 2014], 13% of tarsal tunnel syndromes are the result of varicosities within the tunnel. Is there any consensus as to the treatment? One of my patients certainly has varicosities which were discovered (on MRI) within the tarsal tunnel. Injections, orthotics, ultrasound, physical therapy have failed to reduce this patient's pain. I was considering using a NormaTec device to reduce the venous pressure within the leg as one more non-surgical approach. Any thoughts on this would be well-appreciated.

QUERIES (NON-CLINICAL)
Query: Recent Change in Credit Scoring
 
I read in Kiplinger's Personal Finance (November 2014 issue, page 70) new information about a new change in the FICO credit-score calculations. It states "Medical bills in collection will no longer affect the calculation" of an individual's credit score. "Consumers whose only negative is an unpaid medical debt in collection are not a credit risk."
 
I find it interesting with all the possibilities for the public to renege on a debt, FICO chooses  a medical bill acceptable to ignore. This will likely render turning someone over to collections useless. I am curious what you or others think about this.
