IL Podiatrist To Receive Hospital Award
Childerguild, a charitable organization, will host its 91st annual Ball on November 23rd in Tinley Park. This year’s theme is “Mad Hatter” – a tribute to Disney’s beloved “Alice in Wonderland” film. The evening’s festivities will include the presentation of the Silver Cross Sehring Medal of Excellence for Healthcare to retired podiatrist Reno Caneva, DPM.

Dr. Reno Caneva |
Dr. Caneva is a 1959 graduate of the IL College of Podiatric Medicine (now the Scholl College of Podiatric Medicine), is father of Daryl Caneva, DPM, and grandfather of Andres Caneva, DPM.

INTERNATIONAL PODIATRY IN THE NEWS |
Bahamas Faces Major Shortage of Podiatrists
With one in seven of the population suffering from diabetes, Health Minister Dr. Duane Sands is urging Bahamians to make more healthy lifestyle choices to prevent getting the disease. Speaking at the Bahamas Podiatric Medical Association Conference on Friday, Dr. Sands noted that diabetes, which is now the fifth leading cause of death in the country, has become more prevalent in the Bahamas.
|
Dr. Duane Sands |
It is for this reason, Dr. Sands said, why prevention of foot ulcers is an important priority for podiatric medicine and the diabetic patient in general. “Podiatry services were reinstated in the public sector about one year ago and since that time, approximately 700 patients were treated. They were aged between three months and 98 years.” Sixty percent were treated for a diabetic foot. There were 161 minor surgical cases. However, as noted by Dr. Sands, there is still a major shortage of podiatrists in the country.

PODIATRISTS AND FOOTWEAR |
Shoes are an Investment: CA Podiatrist
Podiatrists say defining a comfortable shoe is totally based on individual preference and the purpose they're trying to serve. "It depends on your foot structure, as well as the purpose for when or how you're going to be using them," says Diane Koshimune, DPM, a podiatrist based in San Jose, CA, and a spokesperson for the American Podiatric Medical Association.
|
Dr. Diane Koshimune |
For example, shoes for plantar fasciitis should have ample arch support (likewise, people with flat feet have fallen arches and need flexible medial posts for stability). People with bunions should look for shoes with a wide toe box and soft, flexible materials so your toes don't feel constricted. "Shoes are an investment," Dr. Koshimune says. "They protect feet from the elements, but they are also the connection between the body and the earth. If the foot has better posture, then the rest of the body will also have better alignment."

Over 700 Doctors Paid More Than $1 Million by Drug, Medical Device Companies
Back in 2013, ProPublica detailed what seemed a stunning development in the pharmaceutical industry's drive to win the prescription pads of the nation's doctors: In just four years, one doctor had earned $1 million giving promotional talks and consulting for drug companies; 21 others had made more than $500,000. Six years later — despite often damning scrutiny from prosecutors and academics — such high earnings have become commonplace.
More than 2,500 physicians have received at least half a million dollars apiece from drugmakers and medical device companies in the past five years alone, a new ProPublica analysis of payment data shows. And that doesn't include money for research or royalties from inventions. More than 700 of those doctors received at least $1 million.

PRACTICE MANAGEMENT TIP OF THE DAY |
7 Conveniences Pateints Expect from Today's Medical Practices - Part 5
Today’s patients expect the same conveniences from medical practices that they receive from restaurants or retailers. If they don’t get them, they will find another doctor. If practices want to compete in today’s healthcare environment, they need to offer patients convenience. Here are the top 7 things patients expect from their doctor’s office.
5. Updated waiting rooms - Free Wi-Fi, coffee, and water are the minimum. A modern design with comfortable furniture and natural lighting will put patients at ease and make them feel valued. A dingy room plastered with warnings and payment notices isn’t exactly patient-friendly.

RESPONSES/COMMENTS (RELEVANT RESEARCH) |
RE: Rising Plasma Copeptin Level Tied to Amputation Risk in Diabetes
From: Leonard A. Levy, DPM, MPH
A study published in Diabetes Care (Potier, L, et al., October 2019) found that during follow-up, the cumulative lower-extremity amputation incidence was higher among patients with type 1 and type 2 diabetes with increasing levels of plasma copeptin, compared with those who had lower plasma copeptin levels. Plasma copeptin, a surrogate marker of vasopressin, is associated with the risk of cardiovascular and renal complications in diabetes. Researchers used a cohort of 5,263 diabetes patients and suggested that improved hydration could reduce the risk.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A |
From: Ira Baum, DPM
Dr. Levy speaks from experience and knowledge, but the practical response from Dr. Purdy would be more effective and efficient. Taking into consideration the healthcare environment today with respect to student debt, years in training, and reimbursement schedules for podiatrists, it doesn’t make a lot of sense to pursue a regional specialty that other medical professionals serve based on a system specialty. There was a time for that type of specialty, but that time has passed. The train has left the station.
The obvious obstacle to transitioning DPM to MD/DO is the solid structure of the podiatric medical systems. Until those systems realize the future of the profession and see the value for those in leadership positions of our associations, educational institutions and boards to change, podiatrists' recognition will remain unchanged, and our war for parity will be never ending. Unfortunately, it is the grassroots podiatrists who will suffer for their inaction. I strongly recommend those in leadership positions to consider these points and begin to explore options and opportunities for change.
Ira Baum, DPM, Miami, FL

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B |
From: James J DiResta, DPM, MPH
Many graduating podiatric medical students would benefit from an Intensive Podiatric Medicine Residency option. Providing this option for our graduates who do not want to be surgeons can have real value IF done correctly. That would work if we were able to engage Dr. Levy in this process in creating such a program, i.e. a 3-year residency curriculum in "podiatric medicine" that could provide for training in areas of general medicine, obstetrics, psychiatry, and make up for those clinical deficiencies our graduates presently have, provide for an allopathic or osteopathic medical school to sponsor the program so the resident can be provided a DO or MD degree, and get the residency in "podiatric medicine" ACGME recognized with accreditation.
I know those are considerable barriers to cross. As a profession, we may need to financially support such an effort in the initial roll out. We would need our students to take and pass the USMLE step 1 which we have known for some time is essential no matter what we do going forward. The completion of this type of residency would provide equal footing with our medical and surgical colleagues for OUR profession "podiatric medicine". Dr. Levy has provided alternative pathways for DPMs to the DO degree before. Perhaps this time, we can obtain a successful solution in a time span equal to that of our podiatric surgically approved programs and that would provide the value many of our graduates want who do not expect to be reconstructive foot and ankle surgeons.
James J DiResta, DPM, MPH, Newburyport, MA
