IA Podiatrist's Diagnosis and Referral Lead to Athlete's Gold Medal
 
It was four weeks to competition day. Cory Sonner of Huxley, then 33, had been lifting mammoth objects, pulling vehicles using muscle alone, flipping huge tires over and over again, and doing other strength-defying acts. He was preparing for his fifth Iowa Games Strongman competition. He felt a pop in his left calf. "I just stopped," he says. He thought there was no chance he'd be able to do similar feats in just 29 days. But Sonner, whose mother is a nurse, promptly sought professional expertise. He consulted with Linda Bratkiewicz, DPM, a podiatrist with The Iowa Clinic, and learned that he had a partially torn left calf muscle.

Dr. Bratkiewicz referred him to The Iowa Clinic's physical therapy team for treatment. The day before the Strongman competition, Bratkiewicz also wrapped Sonner's calf and fitted him with a compression sleeve for the competition. Sonner "flew through" the event repeatedly flipping a 630-pound tractor tire, running a relay race carrying 150-pound bean bags, and pulling a National Guard bullet-proof Hummer 100 feet – and he did it all faster than anyone else in his division. That earned him the gold medal.

Congress Passes Budget Bill That Cuts Medicare Pay in 2025
 
Congress has passed a 2-year budget deal that solves a laundry list of fiscal problems, but creates one for physicians by cutting their Medicare reimbursement in 2025. The House passed the bipartisan measure on October 28, and the Senate followed suit early this morning. President Barack Obama has promised to add his signature.
 
The measure funds the government for 2 more years, averting a shutdown, and raises the debt ceiling through March 2017, averting a default. It allows military and domestic spending to exceed previously set budget limits called sequestration by $80 billion, some of which could make its way to such vital agencies as the National Institutes of Health and the Centers for Disease Control and Prevention. Medicare beneficiaries are spared a dramatic spike in their monthly Part B premiums.

Four years after completing meaningful use stage 1 and receiving the monetary incentive, I am being audited. It has been over 30 days since submitting requested records and I haven't heard from the auditor. What should I expect to happen next? I would appreciate comments from anyone who has either passed or failed.

I was saddened to get word of the passing of Dr. Paul Hershey. Dr. Hershey was the first director of podiatry at Long Island Jewish hospital as well as other institutions. His commitment to podiatric medical education, medial ethics, as well as his commitment to his peers was a model for all of us. I vividly remember that at Nassau County monthly meetings, he always brought in x-rays of complex cases for discussion and everyone learned from him. He will be missed by all who knew him

I recently treated the very same type of condition on a 73 year old male. Although our etiologies are quite different (his problem was the result of an ill-fitting shoe), I tried everything I could think of for two months, and there was virtually no change in the size of the lesion. Finally, I combined Regranex daily dressings with Pletal 100 mg b.i.d., and the ulceration closed in about 6 weeks.

I suggest evaluation for equinus and possibly use of a rocker-sole shoe. The EHL tendon might need to be attached to the 1st metatarsal proximal to the fused MPJ to remove its force on the IPJ. On occasion, I have used neurotoxins (Botox) to temporally paralyze a muscle so that another area could heal.

An LCD (local coverage determinant) #L35013 issued 10/1/15 for 12 states covered by the Medicare administrator, Novitas, has dictated a series of provisions regarding mycotic nail care which are challenging to understand and implement. I guarantee that this LCD will be adopted by all other carriers shortly, so none of us are immune. It is clearly an opportunity by them to restrict such care.
