Take Sports-Related Foot Injuries Seriously: IN Podiatrist

Officials at the American Podiatric Medical Association and American Academy of Podiatric Sports Medicine are warning athletes to take care of their feet when they play fall sports like football and lacrosse.

"Many fall sports athletes look to compete their hardest on the field even after being diagnosed with a foot or ankle injury by a podiatrist or other medical professional," said Dr. Bruce Williams, past president of the podiatric academy, in a statement.

"However, not taking foot injuries such as plantar fasciitis, sprains, and turf toe, seriously and continuing to play a sport on an injured foot or ankle can aggravate and worsen these ailments and delay or significantly worsen the healing process," he said.

AT THE COLLEGES

GA Podiatrist Donates $100,000 to Barry For Anatomy Lab Upgrade

Bradley Bakotic, DPM, DO, and Bako Pathology Services in Alpharetta, Georgia awarded $100,000 to the School of Podiatric Medicine for the renovation and technological upgrade of the Gross Anatomy Lab in Wiegand Hall at Barry University. The new facility will be known as the Bako Pathology Services Anatomy Lab.

The first phase of renovations in the lab is completed: new flooring, first aid and eye wash stations and upgraded temperature systems, among other amenities. When the project is complete in May 2010, the lab will be outfitted with new surgical lighting, video cameras with monitors at each dissecting station, and a fluoroscope.

Dr. Bakotic graduated as valedictorian of the school in 1992 and went on to complete his Doctor of Osteopathic Medicine degree at the New England University College of Osteopathic Medicine. He has served Barry’s School of Podiatric Medicine as adjunct professor, sponsor of alumni receptions and graduation awards and presenter for continuing medical education lectures as well as in other capacities.

AAPPM Fall Workshop Another Smashing Sellout Success

Combine warm weather, a luxury resort, and some of the best practice management experts in the country – and it’s no surprise that over 175 attendees packed the Lago Mar Resort in Ft. Lauderdale this weekend for the American Academy of Podiatric Practice Management’s Fall Practice Management Workshop.

Roundtable at AAPPM Fall Practice Management Workshop

In addition to the well-attended lectures, podiatrists and their staffs interacted with each other and practice management experts in small groups and roundtables. Practitioners shared their successful techniques and concerns with each other. Attendees enthusiatically expressed how valuable and practical this seminar always is.

NY Podiatrist Spooks Patients on Halloween

Patients limping in to see one podiatrist in New Springville last week were greeted at the door by Michael Jackson, the iconic master of fancy footwork. They checked in at the front desk with Minnie Mouse, and were led to an examination room by a woman who bore a striking resemblance either to Sacagawea or Pocahontas. And it was a cigar-chomping Tony Soprano type who swung open the door, ready to check on bunions and corns.

While it may sound far-fetched, this was reality for Staten Island podiatry patients last Friday as Dr. Richard Rouder and his staff decided to get patients into the Halloween spirit as well as administer health care. “Some of them were really shocked when they walked in,” said Dr. Rouder with a chuckle. “At first, they really didn’t recognize some of us. But they all enjoyed it, and everyone got a real good laugh over it.”

Query: Painful Callus, Unresponsive to Conservative Care 

I have a 32 y/o male who has been coming to our practice for several years for B/L painful callus formation. Treatment to date has included debridement, orthotics (custom and accommodative), off-loading pads, Rx lotions, and surgery. None of these treatments have helped in preventing the return of these lesions.

RE: Theoretical Explanation of the Metatarsal Length Pattern

The metatarsal length pattern (MLP) has been implicated in the pathomechanical etiology of a wide range of clinical, chronic conditions. The study of MLPs provides a great challenge for podiatric medicine. A pathomechanical MLP can be thought of as a microtraumatic interaction among the metatarsals.

Each MLP has a unique configuration of the five metatarsal head positions. Each configuration can be described by a unique type of mathematical equation. This gives the clinician a quantitative rationale for pathomechanical diagnosis and treatment of the MLP. Since genes play a pivotal role in human skeletogenesis, the presence of a pathomechanical MLP should call for checking other parts of the body. Also, if a post-operative bunion surgery shows a pathomechanical MLP, there is a good chance of recurrence.

In my current work, I am in a Phase II trial where I am focusing on treating pathomechanical MLPs surgically, using computer-based surgery, and by conservative means, using new modifications of orthotics and inserts, depending on the equations.

RE: Podiatrists and Flu Shots (Elliot Udell, DPM)

We recently discussed whether it was appropriate for podiatrists to manage the systemic illnesses that have pedal manifestations. I believe gout and diabetes were among the examples given; and since podiatrists manage the foot complications due to these illnesses, it seemed prudent to several of our members to be involved, with the internist, in the medical management of these problems.  Others felt like I do, which is that the internist should take care of the medical management, and I will take care of the feet and ankles. It is a gray area.
 
Since influenza has no pedal manifestations, and podiatrists do not treat the symptoms of influenza, podiatrists should not be involved in the care of influenza in any way, including the vaccination process. If there is time to be involved in the legislative process, podiatrists would be much better off working towards podiatric goals.

RE: Pre-Fab Heel Stabilizer (Eric M. Hart, DPM)

The goal of pre-fabricated or custom orthoses in the treatment of pediatric flatfoot is to reduce pain and deformity by reducing excessive pronatory forces across the subtalar joint. The orthoses must be rigid enough to support and realign the subtalar and midtarsal joints, while increasing supinatory torque across the subtalar joint axis. While heel stabilizers can act as a physical block to heel eversion, much more sophisticated and effective devices are available to treat pediatric flatfoot. This article by Paul Scherer in the September 2008 Podiatry Management is an excellent overview of the current literature regarding orthotic treatment of pediatric flatfoot. 

An example of new generation devices is ProLab’s pre-fabricated P3 Kiddythotic which incorporates several functional corrections designed to increase supinatory torque across the STJ axis and limit excessive collapse of the medial arch. These include a medial flange, deep heel cup, medial heel skive, and a rearfoot post.

We would be more than happy to let your client come down here and take all the ER call he wants. The only thing out of the ER is uninsured, alcohol-induced trauma. Nothing like getting out of bed at 2 AM  to operate on a drunk.
