Armstrong to Receive Limb Salvage Award

David G. Armstrong, DPM, PhD, professor of surgery, chair of research and associate dean of the Dr. William M. Scholl College of Podiatric Medicine at Rosalind Franklin University of Medicine and Science in Chicago, is the inaugural recipient of the Georgetown Distinguished Achievement Award in Diabetic Limb Salvage, announced Christopher E. Attinger, MD, co-chair of the Georgetown University Hospital Conference on Diabetic Limb Salvage, A Team Approach, and director of the Wound Healing Center at Georgetown University Hospital in Washington, DC. The award will be presented at the Georgetown conference in Washington, set for September 27-29, 2007, at the JW Marriott.

“The Distinguished Achievement Award recognizes outstanding work in the field of limb preservation, especially that involving a team approach, and Dr. Armstrong is known nationally and internationally as a pioneering researcher, dedicated clinician and tireless advocate and educator on limb salvage,” Dr. Attinger said. Dr. Armstrong also is director of the Center for Lower Extremity Ambulatory Research (CLEAR) at Advocate’s Lutheran General Hospital in Chicago and co-founder of the renowned National Center for Limb Preservation in Chicago.

NYCPM Launches Clinical Competence Training Program in New Clinical Skills Center

The New York College of Podiatric Medicine (NYCPM) recently launched a new Clinical Competence Training Program employing trained professional actors as “standardized patients” who accurately simulate a multitude of medical conditions or symptoms of a disease and provide feedback to the students on their clinical skills.

Barbara Haas, the “standardized patient”, a trained professional actor, is flanked by (l. to r.) Michael J. Trepal, DPM, VP, Academic Affairs and Dean; Crystal Gonzalez, Class of 08; Louis L. The Clinical Competence Training Program at NYCPM, geared to improving students’ excellence in history-taking, communication and physical examination, is being conducted in the College’s new Clinical Skills Center. Second-year students work one-on-one in the Center’s examination rooms, learning to take the standardized patient’s medical history and to conduct an accurate and complete physical assessment in a humanistic manner. The College also has plans to develop CME courses in this area.

NV Cross Country Champ Credits WA Podiatrist

Pahrump Valley High School sophomore McKenzie Dean ran what her coach called "her smartest race of the year" to win the Nevada Class 3-A girls cross-country title here Saturday. Dean clocked a time of 21 minutes 31.23 seconds over a five-kilometer (3.1 mile) course at Veterans Memorial Park to pace the field among the Class 3-A girls.

Dean said Dr. David Gent, a podiatrist from Washington State, made a special insert for her shoes. "During track season I had real bad knee pains because of my growth rate," Dean said. "He cast my feet. It helped me so much, it supports your arch."

Query: Chronic Tibial Sesamoiditis and HAV

I have a middle-aged woman whom I have treated well over a year conservatively for a chronic tibial sesamoiditis. I have tried: orthotics, NSAIDs, PT, Steroid injections, and shoe modification. She gets periodic relief. The sesamoid on x-ray is hypertrophied, bi-partite and plantarly prominent (Axial sesamoid view). Additionally, the patient has a symptomatic, moderate HAV deformity. (IM angle approximately 12-13 degrees).

IWhat is the best way to correct the HAV deformity while either removing or planing the tibial sesamoid? Should this be done through one incision or 2 (dorsal and medioplantar)? Are there any special considerations with removing the sesamoid and a bunionectomy with osteotomy?

RE: Sculptra (Jim Fisher, DPM)

To date there are no published papers for use of Sculptra in the foot. Sculptra comes packaged with rwo vials with the poly L lactic acid inside. I mix the powder form with 5cc's of sterile water and 1cc of 1% lidocaine plain. This needs to be done at least two hours prior to its use but 24 hours is best. Next, a PT nerve block is performed. The Sculptra is injected with no smaller than a 22-gauge needle or else the crylstals will clog the needle. You will find that multiple needles will need to be used. I usually inject 3cc's per foot, high pressure areas get the most substance.

You must tell the patient that they will get a quick plump up but then the water will be resorbed and they will not feel any cushioning. They may feel as if they had a poor result. Actually, at about 6 weeks if there is no reponse than a second treatment may need to be done. The lactic acid will work to build collagen and results can take quite a frew weeks. This usually lasts 8-9 months in the foot.

If a patient wants a quicker solution, try Restylane or Radiesse.

I would agree with Dr. Habershaw and keep it simple. An IPJ arthroplasty, although not done commonly, can be very simple and effective in a situation such as this.
