A single CPT 28122 (partial excision [craterization, saucerization, sequestrectomy, or diaphysectomy] bone [e.g., osteomyelitis or bossing]; tarsal or metatarsal, except talus or calcaneus) is the code one would bill when performing a 1st metatarsal-cuneiform joint exostectomy. The procedure involves resection of a bony prominence, usually dorsally located, abo but the joint involving contiguous bones.
A "reverse Austin tailor's bunionectomy" is a distal osteotomy (with or without fixation) of the 5th metatarsal with remodeling of any hypertrophied bone present. The procedure is CPT 28308. This is not CPT classified as a "bunionectomy" or "mini-bunionectomy" or "mini- Austin" - it is an osteotomy.
There are some who feel that, in addition to the CPT 28308 procedure, an ostectomy procedure, like CPT 28122 or CPT 28110, should be added, but, in fact, the Correct Coding Initiative bundles CPT 28122 (comprehensive) and CPT 28308 (component); the Correct Coding Initiative bundles CPT 28110 (component) and CPT 28308 (comprehensive); the American Academy of Orthopaedic Surgeons Complete Global Service Data for Orthopaedic Surgery states that CPT 28122 is included in the global service package for CPT 28308; and the American Podiatric Medical Association's "Report of the Task Force on Global Surgical Services" states that a partial metatarsal ostectomy is an incidental procedure to CPT 28308.

I’ve been using the Panacos graft and the Bleomycin treatments ever since I first started practicing. I list these two together because they attempt to accomplish the exact same thing with somewhat different techniques.
Bleomycin technique: The Bleomycin treatment is my primary treatment for verrucae vulgaris lesions, and I currently have a 99.7% success rate over 28 years and hundreds of cases with the added benefit that if the person has multiple verrucae, treating just the one lesion will allow resolution of all of them in the course of 5-6 weeks. The procedure entails using the body’s own immune system to kill the wart and follows the path of driving some of the warty material into the dermis, where the body will identify it and raise killer T-cells to the...

Before trying Intralesional Cidofovir for refractory warts, I recommend considering pulsed-dye laser treatment which has been found to be a safe effective treatment for recalcitrant warts. My office has treated many thousands of patients with warts using the pulsed-dye laser and receive referrals from dermatologists, podiatrists, and primary care physicians for treatment after they have failed various other treatment modalities. We have been able to resolve more than 95% of all verrucae with the pulsed-dye laser. We actually use pulsed-dye laser as the initial treatment for almost all warts.
Very few podiatrists own a pulsed-dye laser. Many dermatologists do have a pulsed-dye laser; however, my experience is that almost none of them use it for the treatment of warts; it is predominantly used for cosmetic procedures. I think that is because the machine takes about 15 minutes to "warm up" and it is so much easier for them to use liquid nitrogen. Also, it is a very expensive device costing anywhere from $40,000 used to $100,000+ new with high maintenance costs. In addition, there is no additional payment. The appropriate code is the usual CPT 17110 code.
If you have a recalcitrant wart you might want to see if there is a podiatrist or dermatologist in your area who owns a pulsed-dye laser and has experience using it for warts. The laser is very effective with very mild discomfort. Less than 1% of treatments require anesthesia. It can also be used during pregnancy, and most children can tolerate it.

I want to thank Dr. Kesselman for his kind comments, insightful observations, and salient questions concerning the future of podiatric medical education. I don’t think I’ve ever met Dr. Kesselman, but I can unequivocally say, “He gets it!” He is majorly prophetic in his predictions about the future and what we need to do in order to preserve our rich heritage while not losing our essence. It is not unequivocally necessary to retain only the initials DPM after our names to maintain the mandate handed to us through the years to be who we are and continue to be it well. When DSC transitioned to DPM, holders of the former degree did not have to undergo extended periods in sweat lodges to purify themselves of the DSC degree and become DPMs before exiting the ritual cleansing of initials that had become anathema. Some chiropodists preferred to retain the DSC initials but everyone in our profession retained the traits and identity of the chiropodist before signing their name as DPM for the first time. If anything, the initial change spurred us on to become better at what we did rather than distancing us from our roots. We seem to have survived the change quite nicely I might add.
It seems to be time for another major step in our evolution. The next generation to whom we will pass the torch has asked the post-nominal letters be changed to signify an unrestricted license. It is quite obvious from the decline in enrollment at podiatry colleges that the younger generation is not satisfied with a restricted...
