NY Podiatrist Provides Advice on Preventing Blisters
Having to sideline yourself because your feet hurt has got to be the most humiliating secret party foul, sadder even than a spilt drink you paid for yourself. The key is prevention, says Dr. Jackie Sutera, podiatrist and Vionic Innovation Lab member.
|
|
Dr. Jacqueline Sutera |
"The best advice I can give is to wear new shoes at home for short periods of time before wearing them all day," Dr. Sutera says. "This gives you a chance to notice any sensitive spots and also allows the shoe to 'give' a little bit. I do not encourage people to 'break in' shoes. They should fit well at the store, and if they don't, don't buy them! But new shoes are always a bit stiff/tight, so wearing them a little while at a time definitely helps. If you do have sensitive spots, pad them with moleskin or Band-aids."

ND Podiatrist to Join Jamestown Regional Medical Center
Dr. Rachael Renschler will join Jamestown Regional Medical Center as its new podiatrist beginning in January. Renschler will begin seeing patients in late January. Renschler attended Des Moines University in Iowa, where she received her Doctor of Podiatric Medicine.
|
|
Dr. Rachael Renschler |
She recently completed her podiatry residency at Iowa Methodist Medical Center in Des Moines, IA. Although Renschler is skilled in foot and ankle procedures, she will also expand her services into more non-surgical care. "Just because you don't need surgery does not mean I don't have a service that will benefit you and the health of your feet," Renschler said.

DC Podiatrist Sees Big Potential for 3D- Printed Shoes
A limited number of new Adidas’s new 3-D-printed sneakers, called the “3D Runner,” went on sale in New York City on Thursday. The $333 sneakers are constructed with a web structure that’s thicker in areas that need more force absorption and thinner in areas that won’t experience as much force, and have more elasticity than your typical shoe. That idea of a totally customized shoe for your foot shape, made easier with the help of 3-D printing, is something Lee Firestone, DPM, a DC podiatrist with Foot & Ankle Specialists of the Mid-Atlantic, said he’d be eager to see. “We have a population of patients who would love to have orthotics—say somebody was training for a race and they needed those orthotics within a day or two—the benefit of being able to custom-design an orthotic for somebody and have it printed in the office and ready for them within a day or two or three—athletes would love that,”
|
|
Dr. Lee Firestone |
Firestone also sees a big potential for how 3-D printed shoes could assist his diabetic patients, who often experience nerve damage in their feet, by providing them with custom shoes that don’t rub on the unique pressure points their feet have. Additionally, those who suffer from bunions or hammertoes could potentially find relief with a shoe designed specifically for their foot shape. “Think about how many people—especially women—are looking for a stylish shoe that accommodates their bunion deformity—and they’re stuck,” says Firestone. Those women can chose between the cute shoes that hurt their feet or clunky orthopedics. But with 3-D printing, they could someday have the same stylish silhouette customized with a wider toe box that wouldn’t contribute to bunions.

CA Podiatrist Indicted for Allegedly Defrauding VA
Federal officials charged a VA doctor and the owner of a Sacramento shoe store with fraud, Friday, saying they attempted to cheat the Department of Veterans Affairs in a scheme involving the delivery of nearly $1.7 million worth of orthopedic and therapeutic shoes for veterans. Anthony Lazzarino, 66, former chief of podiatry in the VA’s Northern California health care system, and Peter Wong, 58, founder of Sunrise Shoes in Sacramento, were charged in the scheme.
A grand jury indictment said the two men teamed up to bill the VA for nearly $1.7 million worth of specialized shoes, some of them costing as much as $1,682 a pair, during a six-year period starting in 2009. In fact, the veterans only received “off-the-shelf” products from Sunrise Shoes.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
10 Collection Tactics for High-Deductible Patients- Part 7
Patient collections are much more than collecting $10 co-pays: those days are long gone. Patients are paying a greater percentage of what you get paid and will be paying an ever higher percentage in the years to come. Smart and consistent patient collections tactics must be a part of every practice's financial strategy. Though the rate of healthcare spending may have slowed, the share absorbed by patients has done the opposite. Here are a few basics for improving your patient collections.
7. Consider credit card on file: With appropriate safeguards, practices can keep patients' credit cards on file and charge them for patient balances as long as the patient has given you written permission to do so. You do not want the liability of keeping their credit card information onsite, so use a certified and secure third-party to retain the information.

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 | |
RE: Time for Podiatrists to be listed as Castle Connolly Top Doctors
From: Michael Loshigian, DPM
Well It's that time of the year again when Castle Connolly is open for nominations for its published lists of Top Doctors across the country. Every year, I nominate doctors that I have worked with closely and know to be among the best in their area of their specialty. Every year, it is a bittersweet experience as I cannot nominate deserving doctors of my own specialty. For as long as I can remember, podiatry has been excluded from this publication.
I have included my letter below as an example...

I also have Practice Fusion. The easiest way I have found to upload images is to take a “screen shot” of my computer while viewing the image, then upload it into the Practice Fusion documents section.
Dave Williams, DPM, El Paso, TX
I would keep them separate. Be sure that each program is backed up daily, securely, and off-site. Then, run periodic checks to make sure the back-ups actually worked and can be recovered.

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 3 | |
From: Robert Kornfeld, DPM
Here we go again. It is truly maddening to me that the "evidence" the status quo is still looking for are the effects of a drug vs. a placebo. And now, we are substituting nutrients and measuring their effects as if they are drugs. I opine that the entire evidence-based model fails miserably in its understanding of what we should be measuring. I've said this before and I'll say it again, all of these studies conveniently ignore the patient and their unique physiology, genetics, and epigenetics. If you are going to use nutraceuticals in the treatment of diabetic neuropathy, then there must be a deeply layered protocol put into action prior to the prescription of nutraceuticals.
We first need to educate the patient, optimize the epigenetic influence (a healthy patient-specific diet, exercise, stress control, sleep hygiene, hydration, communication skills, etc.) and work to facilitate...

From: Don Peacock, DPM
I've had the opportunity to perform the Katzen modified version of the MIS Wilson osteotomy. The procedure is very effective in some patients. I use the procedure in mild to moderate bunion deformities with mild PASA and a relatively small bunion/ "bump". I do agree with Dr. Greenberg and Kass with respect to head-to-head x-ray evaluations. The excellent surgical result from Dr. Cohen is in the early post-op stages and it is obvious that the patient is guarding in the post-op. Here is a patient whom I performed the procedure on, including a weight-bearing pre-op AP view along with a six week post-op view.
|
|
Pre- and post-op weight-bearing AP views |
In these views, we're seeing the deformity correction further out and the patient is not guarding as much in the post-operative views. Secondary bone healing is seen at this stage and good anatomical, clinical, and subjective results were achieved. The pre-op IM angle is 13 and the HA angle is 22. The post-op IM angle is 8 and the HA angle is 11.

From: Harry Goldsmith, DPM
Jeff Stone passed away several weeks ago. For those of us who knew Jeff, it came as quite a shock. We always assumed he would show up at the next seminar, sitting in the exhibit hall, shoe and sock off, trying to find a neuroma in his foot using an ultrasound probe. What happened in his life to reduce him to shoeless Jeff Stone? I often wondered about that. I also wondered where the neuroma was. Why didn’t Jeff take care of it? And how much would I have to pay him to inject it in the exhibit hall? A dollar? Maybe.
Jeff had what could only be called “an active sense of humor.” You never left his presence without a joke to two, maybe a ribbing, always a smile. I have three Jeff Stone stories.
Story 1: The first time I met Jeff was during a...
